流感嗜血杆菌分布及耐药性分析 被引量:14

摘要 目的了解临床分离流感嗜血杆菌(Hi)的分布及耐药情况,为临床合理使用抗菌药物、预防和控制感染提供依据。方法收集中南大学湘雅医院2010~2012年各类临床标本Hi分离株187株,用K-B纸片扩散法检测Hi对常用的14种抗菌药物的敏感性,用头孢硝噻吩纸片法检测产β-内酰胺酶情况。结果 187株Hi主要分离自呼吸内科(41.17%),以儿童(21.39%)和老年(39.57%)患者居多。冬、春两季分离率较高,分别为36.90%(69/187)和30.48%(57/187)。Hi对氨苄西林、复方磺胺甲噁唑和头孢呋辛的耐药率较高,依次为81.3%、57.2%和38.5%;对氨苄西林/舒巴坦、头孢曲松、洛美沙星、左氟氧沙星、四环素、头孢噻肟、环丙沙星和阿奇霉素敏感率均>60%。未成年人分离株对环丙沙星、氧氟沙星、左氟氧沙星敏感率(82.35%、76.47%、97.06%)高于成人分离株(49.57%、47.89%、79.87%),χ2值分别为19.629、14.529、10.728,P均<0.05;对头孢曲松的敏感率(85.29%)低于成人分离株(94.12%),χ2=4.076,P<0.05。产β-内酰胺酶株检出率为64.17%,β-内酰胺酶阴性氨苄西林耐药(BLNAR)菌株22株。结论 Hi对氨苄西林和复方磺胺甲噁唑耐药严重,而β-内酰胺酶抑制剂、喹诺酮类、第2、3代头孢类药物、阿奇霉素可作为优先选择药物。
