摘要
目的观察双侧钢板固定术治疗Schatzker Ⅴ、Ⅵ型胫骨平台骨折的临床疗效。方法 80例Schatzker Ⅴ、Ⅵ型胫骨平台骨折骨折患者根据手术方法分为双侧组和单侧组,单侧组采用单侧锁定加压钢板固定术治疗,双侧组采用内外侧双钢板固定术治疗,对术后骨性愈合时间、骨折复位情况、膝关节功能及术后并发症进行评价。结果两组患者均获得临床愈合,双侧组愈合时间显著低于单侧组(P<0.05)。双侧组术后骨折复位率为100.0%,单侧组术后骨折复位率为82.5%,两组相比较差异有统计学意义(P<0.05)。双侧组术后发症发生率为15.0%,单侧组为42.5%,两组比较差异有统计学意义(P<0.05)。双侧组膝关节功能评估优良率为87.5%(35/40),显著高于单侧组的57.5%(23/40),两组比较差异有统计学意义(P<0.05)。结论内外侧双钢板固定术治疗Schatzker Ⅴ、Ⅵ型胫骨平台骨折是一种安全有效的治疗方法,值得临床应用。
Objective To study clinical efficacy of fixation of bilateral steel plate for treating the patients with SchatzkerⅤand Ⅵ tibial plateau fracture.Methods 80 cases of patients with SchatzkerⅤ and Ⅵ tibial plateau fracture were divided into bilateral group and unilateral group according to surgical methods.After surgery the fracture healing time,fractures situation,rehabilitation of knee function and postoperative complications were observed.Results The patients of two groups achieved clinical healing,the fracture healing time of the bilateral group was significantly lower than the unilateral group（P0.05）.The reduction rate of postoperative fractures of the bilateral group was 100.0%,the unilateral group was 82.5%,compared the difference was significant（P0.05）.The postoperative complications of bilateral group was 15.0%,it was significantly lower than that of the unilateral group（42.5%）,the difference was significant（P0.05）.The good rate of rehabilitation of knee function of bilateral group was 87.5%（35/40）,it was significantly higher than that of the unilateral group（57.5%）,the difference was significant（P0.05）.Conclusion The fixation of bilateral steel plate is a safe and effective treatment for the patients with SchatzkerⅤ and Ⅵ tibial plateau fracture,and it is worthy of clinical application.
