摘要
目的对比观察单纯后路病灶清除植骨内固定术和一期前路病灶清除椎体间植骨+后路植骨内固定术治疗胸腰椎结核的优势及疗效。方法回顾性分析深圳市龙华新区龙华人民医院自2008年1月至2012年1月收治的46例胸腰椎结核患者的临床资料,将其中接受单纯后路病灶清除植骨内固定术的26例作为观察组,接受一期前路病灶清除椎体间植骨+后路植骨内固定术的20例作为对照组。对两组患者围术期及治疗前后相关指标进行比较。结果观察组的手术时间、术中出血量、输血量、引流量、抗生素应用天数、住院天数等均优于对照组,各指标比较差异均有统计学意义(P<0.05)。但两组治疗前和治疗半年后的视觉模拟评分、红细胞沉降率和Cobb角等均无显著差别,各指标比较差异均无统计学意义(P>0.05)。结论单纯后路病灶清除植骨内固定术和一期前路病灶清除椎体间植骨+后路植骨内固定术对胸腰部结核具有相同的疗效,但前者具有手术时间短、术中出血少、住院天数少的优势。
Objective To explore advantage and curative effect of isolated posterior debridement and internal fixation for thoracolumbar tuberculosis and anterior focus clearance ＋ posterior approach internal fixation with bone fusion for thoracolumbar spinal tuberculosis.Methods From January 2008 to January 2012,46 thoracolumbar tuberculosis patients in Shenzhen Longhua New District Longhua People＇ s Hospital were retrospectively analyzed,26 cases with one-stage posterior debridement,fusion and instrumentation for thoracolumbar tuberculosis were as observation group,20 cases with anterior focus clearance ＋ posterior approach internal fixation with bone fusion were as control group.perioperative indicators and related indicators before and after treatment of the two groups were compared.Results The operation time,amount of bleeding,intraoperative blood transfusions,lead flow,antibiotics application time and the number of days in hospital of the observation group were statistically significantly better than the control group（P 0.05）.The VAS pain score,sedimentation and Cobb Angle of the two groups before treatment and six months after treatment werenot statistically significantly different（P 0.05）.Conclusion Isolated posterior debridement and internalfixation for thoracolumbar tuberculosis and anterior focus clearance ＋ posterior approach internal fixation with bone fusion for thoracolumbar spinal tuberculosis have the same effect,but the former has the advantage of shorter operation time,less intraoperative bleeding,and less days in hospital.
