CT引导下神经根射频加神经根周围注射倍他米松治疗老年腰椎管狭窄症疗效观察 被引量:4

摘要 目的探讨CT引导下精确定位神经根后,应用射频热凝靶点消融加神经根周围注射倍他米松治疗老年腰椎管狭窄症的临床疗效。方法 98例老年腰椎管狭窄症患者随机分为两组:Ⅰ组52例,单纯接受药物治疗;Ⅱ组46例,在药物治疗基础上,在CT影像导引下行腰椎管狭窄节段神经根射频热凝治疗及相应椎间孔内神经根周围注射倍他米松治疗。治疗180天后通过JOA评分评估两组临床疗效。结果治疗180天后,根据JOA评分标准:Ⅰ组患者优良率44.2%,有效率63.5%;Ⅱ组患者优良率65.2%,有效率82.6%。Ⅱ组患者的优良率和有效率优于Ⅰ组(P<0.05)。结论射频热凝靶点消融加神经根周围药物注射是老年腰椎管狭窄症的一种微创、可行、安全的治疗方法。 Objective To observe the therapeutic effect of senile lumbar spinal canal stenosis with CT - guided minimally invasive transfo- raminal pulsed radiofrequency and epidural injections with betamethasone applied to nerve root. Methods A total of 98 old patients with lumbar spinal canal stenosis according to strict inclusive criteria were included into this study and divided into two groups. 52 cases of group I were given conservative medication. 46 cases of group II underwent CT - guided minimally invasive transforaminal pulsed radiofrequency and epidural injec- tions with betamethasone 5mg applied to nerve root, in addition to conservative medication. We compared the JOA scores of each patients before and 180 days after treatment. Results At the follow - up after 180 days of treatment, The good rate and effective rate were 44.2% and 63.5 % re- spectively. Compared to group I, the good rate and effective rate in group II were 65.2% and 82.6% respectively, which were significant better than group I. Conclusion CT- guided minimally invasive transforaminal pulsed radiofrequency and epidural injections with betamethasone ap- plied to nerve root may be a microinvasive, feasible and relatively safe intervention technique for senile lumbar spinal canal stenosis.
