摘要
目的 :了解肾上腺糖皮质激素 (激素 )对大鼠阿霉素肾病肾小球损害有无加重作用。方法 :40只雄性SD大鼠 ,随机分成正常对照组 (A组 )、阿霉素组 (B组 )、阿霉毒与激素组 (C组 )及激素组 (D组 )。 B组与 C组一次性经尾静脉注射阿霉素 7mg/ kg。各组于实验的第 16周及 2 8周分别取 5只大鼠的肾脏与血液 ,观察肾脏病理改变 ,测定尿蛋白、血清肌酐、甘油三酯、总胆固醇等指标的变化。结果 :注射阿霉素 1周后尿蛋白开始升高 ,4周后达到 16 8.45±76 .2 8mg/ d。使用激素 12~ 2 4周 (注射阿霉素 16~ 2 8周 )后 :B组尿蛋白、血肌酐、甘油三酯及胆固醇都显著高于 A组 ,同组第 2 8周与第 16周比较 ,尿蛋白、血肌酐升高差异有显著性 ,血浆清蛋白降低有显著差异。C组与 B组比较 ,第16周及第 2 8周尿蛋白、血甘油三酯及胆固醇显著升高 ,第 16周的血肌酐明显升高。 D组尿蛋白、甘油三酯及胆固醇显著高于 A组。B组肾脏病理改变的积分显著高于 A组 ,且第 2 8周显著高于第 16周 ;C组第 16周显著高于 B组 ;D组的第 16周和第 2 8周都显著高于 B组。结论 :长期使用大剂量激素可以诱发正常大鼠发生肾小球损害 ,而用于大鼠阿霉素肾病模型则可加速肾小球损害的进程。
Objective:Whether glucocorticoid exert an acceleration effect or not on glomerulus losses in rats with adriamycin nephropathy was studied.Methods:40 male SD rats were randomly divided into A(control),B(adriamycin),C(adriamycin plus glucocorticoid) and D(glucocorticoid alone)groups.Rats in groups B and C were injected with a single dose of 7mg/kg adriamycin via caudal vein.Urine protein excretion,serum creatinine,serum triglyceride,total cholesterone and renal pathologic changes were observed in 5 rats of every group at the 16th week,the 28th week respectively. Results:Urine protein excretion rate was elevated significantly within one week after injection with adriamycin,and increased to 168.45±76.28mg/d after four weeks. After treatment with glucocorticoid for 12 weeks and 24 weeks(i.e.16 weeks and 28 weeks after injected with adriamycin), the levels of urine protein excretion,serum creatinine,serum triglyceride and total cholesterone in group B were significantly higher than that in group A.Compared with group B, urine protein excretion,serum creatinine,serum triglyceride and total cholesterone in group C were elevated markedly .Parameters from group D varied,urine protein ,serum triglyceride and total cholesterone were higher than group A.In respect of renal pathologic changes scores, group B higher than group A, group C at 16th weeks higher than group B,group D at the both 16th week and 28th week higher than group B.Conclusions: High doses glucocorticoid in long term use can induce glomerlus damage in healthy rats. It can also accelerate glomerulus losses in rats with adriamycin nephropathy.
