摘要
目的:分析我院门诊处方书写不合格和不合理用药情况,以提高我院处方书写质量,促进合理用药,保障医疗安全。方法:以随机抽样法抽取我院2012年1～12月门诊处方111 119张(不包括麻醉药品和精神药品处方),依据《处方管理办法》、药品说明书及《医院处方点评管理规范(试行)》,对处方进行统计和分析。结果:不合理处方共2 057张,其中处方前记419张(20.4%),处方正文1 412张(68.6%),处方后记226张(11%)。处方前记和后记表现在书写不合格,处方正文不合理主要表现在书写不合格、选药不当、联用不当、重复用药、用法用量及抗菌药物使用不合理等。结论:我院的门诊处方还存在许多书写问题和不合理用药的情况。药师应加强对处方的审核,及时发现并纠正不合理用药,确保患者用药安全、经济、有效。
Objective：To analyze the unqualified prescriptions in formal writing and cases in unreasonable medicine in the out-patient clinics of our hospital, to improve the quality of prescriptions ＇formal writing in our hospital, promote to use drugs properly. Methods ：To use random sampling method, to sample 111119 pieces of prescriptions in this hospital from January to December in 2012. （except anaesthetic and psychotropic drugs ）, according to（Prescription management meth- od）, package insert of drugs and（The norms of hospital prescription＇s evaluation and management（ trail implementation ） ）, to conduct statistics and analysis for those prescriptions. Results：There are 2057 pieces unqualified prescription. 419 pieces of all are existing mistakes with the first part of prescription（ 20.4% ） , 1412 pieces of all are existing mistakes with pre- scription＇s text （68.6）,and 226 pieces of all are existing mistakes with last part of prescription（ 11% ）, there are several mistakes in formal writing of prescription, especially showed both in first part and last part of prescription, problems in pre- scription text mainly showed in the formal writing, selecting medicine improperly, combined using of drugs improper, use sin- gle drug repeatedly, usage and dosage and abusing antibacterial ect. Conclusion：There are still exist a lot of writing prob- lems and unreasonable medicine cases in the out-patient prescription s in our hospital. Pharmacists should enhance the ex- amination toward prescriptions, similarly, find out and modify those unreasonable prescriptions, guarantee give patients a safe, economic and effective medication.
