摘要
目的探讨2种不同手术方法治疗翼状胬肉的临床治疗效果。方法将2008年11月至2011年10月收治的翼状胬肉患者158例170只眼按照随机数字表法分为2组。对照组58例62只眼采用单纯翼状胬肉切除术,观察组100例108只眼采用翼状胬肉切除联合带自体角膜缘干细胞的结膜瓣转移术,观察2组患者的临床治疗效果。结果观察组患者的角膜上皮修复时间为(3.22±0.75)d,明显低于对照组的(5.36±0.89)d,2组比较差异有统计学意义(P<0.01)。观察组患者的临床治疗痊愈率(87.04%)明显高于对照组(61.29%),术后胬肉复发率(4.63%)也明显低于对照组(17.74%),组间差异均有统计学意义(P<0.01)。结论翼状胬肉切除联合带自体角膜缘干细胞的结膜瓣转移术能明显提高临床治愈率,缩短角膜上皮愈合时间,降低术后复发率,临床操作简便,无术后并发症,值得临床推广应用。
OBJECTIVE To investigate the clinical effect of two surgery methods for pterygium. METHODS Between November 2008 to October 2011, 158 patients （170 eyes） with pterygium were treated in our department. They were divided into two groups according to the random digits table. The 58 patients （62 eyes） in the control group were treated with pure pterygium excision. The 100 patients （108 eyes） in the observation group were treated with pterygium excision combined with tunica conjunctiva flap autotransplantation which contained limbus corneae stem cell. The clinical results of the two groups were observed. RESULTS In the observation group, the mean repaired time of the corneal epithelium was （3.22±0.75） d, which was significantly lower than that of （5.36±0.89） d of the control group （P〈0.05）. The total cure rate of the observation group （87.04%） was significantly higher than that of the control group （61.29%）, with lower recurrence rate （4.63% vs. 17.74%） （P〈0.01）. CONCLUSIONS Ptery- gium excision combined with tunica conjunctiva flap autotransplantation which contained limbus eorneae stem cell was an effective method for treating pterygium. It had low recurrence rate with good clinical result. It also reduced the repaired time of the corneal epithelium with high cure rate.
