摘要 目的回顾性分析胚胎移植日雌激素水平的变化对IVF-ET妊娠结局的影响。方法分析196例行IVF-ET患者资料,于HCG日及ET日测外周血E2值。分为E2值增高组和降低组,降低组又根据E2值下降幅度分为A、B、C、D、E组,比较各组的胚胎种植率和临床妊娠率等。结果E2值增高组和降低组的胚胎种植率和临床妊娠率比较差异有显著性(P<0.05)。A组、B组、C组、D组各组间胚胎种植率和临床妊娠率的比较差异无显著性(P>0.05),且以上各组的种植率和妊娠率均高于E组,E组与A组、B组、C组、D组妊娠率比较差异均有显著性(P<0.05)。结论ET日E2下降幅度>50%时会引起胚胎种植率和临床妊娠率的降低,可考虑给予雌激素的补充。 Objective To study the effect of serum estradiol level on the day of embryo transfer on in-vitro fertilization (IVF-ET) outcome. Methods Total of 196 cases of IVF-ET cycles were evaluated retrospectively. Serum estadiol was measured on the day of hCG administration and the day of embryo transfer (ET). They were divided into increase group and decline group. The decline group was further divided into group A, group B, group C, group D, and group E according to the percent of estradiol decline range. Implantation rate and clinical pregnancy rate were compared among the groups. Results There were statistically significant differences between the implantation rate and clinical pregnan- cy rate in the increase group and decline group (P〈0.05). There were no statistically significant differences between the implantation rate and clinical pregnancy rate in the decline group which were group A, B, C, D (P〉0.05), and were higher than group E. The implantation rate and clinical pregnancy rate were significant differences in statistic between the group E with group A, B, C, and D (P〈0.05). Conclusion When the down-range of the serum estradiol is more than 50% from hCG day to ET day, the embryo implantation rate and clinical pregnancy rate were lower. Therefore, the proper supplementation of estradiol may be necessary during luteal phase.
