摘要
目的探讨帕瑞昔布钠超前给药预防瑞芬太尼全麻患者苏醒期痛觉过敏的疗效和安全性。方法选择100例2011年6月—2012年12月在该院行择期手术施行全身麻醉患者,ASA分级I~Ⅱ级,完全随机均分为A组(帕瑞昔布钠组)和B组(生理盐水组),每组50例。在手术结束前30 min分别予静脉注射帕瑞昔布钠40 mg或生理盐水2 mL。观察记录拔管前、拔管即刻、拔管后1,3,5 min MAP和HR;记录拔管时间、苏醒时间、瑞芬太尼用量、麻醉苏醒期恶心呕吐、躁动、痛觉过敏评分等。结果两组患者拔管时间及苏醒时间比较差异无统计学意义(P>0.05)。B组拔管后1,3 min MAP、HR较A组和拔管即刻明显升高,差异有统计学意义(P<0.05)。B组患者苏醒期痛觉过敏指数6~9分发病率明显高于A组,差异有统计学意义(P<0.05)。结论帕瑞昔布钠超前给药预防瑞芬太尼全麻患者苏醒期痛觉过敏是安全有效的,拔管期间血流动力学稳定,适合临床推广。
Objective To explore the efficacy and safety of parecoxib sodium prevention of hyperalgesia in waking period by using remifentanil. Methods A hundred patients of general anesthesia in the hospital for elective surgery from June 2011 to December 2012, ASA Ⅰ-Ⅱ , were randomly assigned into group A (parecoxib sodium group, n = 50) and group B (saline group, n = 50). Thirty minutes before the end of the surgery , pareeoxib sodium 40mg were given by intravenous in group A and saline 2ml were given at the same time in group B. MAP and HR were observed and recorded before extubation ,extubation and the 1,3,5 min after extubation. The extubation time, recovery time, the dose of remifentanil anesthesia recovery period of nausea and vomiting, restlessness, hyperalgesia score were Recorded. Results There were no significant differences at extubation time and awake time between group A and group B ( P 〉 0.05 ). MAP and HR were significantly increased in the greater way after extubation 1,3 min in group B than that in group A and at extubation, and the difference was statistically significant( P 〈 0.05 ). The incidence of waking period hyperalgesia index 6 to 9 points was signifi-cantly higher in group B than that in group A, and the difference was statistically significant ( P 〈 0.05 ). Conclusions Advance admin- istration of parecoxib sodium preventing the waking period hyperalgesia is safe and effective with anesthesia by using remifentanil. The he-modynamic is stable during extubation.
