摘要
目的:探讨腹膜透析患者血清中性粒细胞明胶酶相关载脂蛋白(NGAL)水平与患者体内炎症指标的关系,评估强化透析方案可否改善患者体内微炎症状态。方法:纳入医院腹膜透析患者80例,同时纳入60名健康人为对照组,收集患者及健康对照人群的临床和生化学资料及血清NGAL、CRP、TNF-α、IL-6等情况,做血清NAGL与炎症因子相关性分析。比较强化腹膜透析方案前后CRP、IL-6、NGAL、TNF-α及Kt/V比值的变化。结果:腹膜透析患者其血清NGAL水平为(655.22±103.8)ng/mL,健康对照人群为(49.14±7.20)ng/mL,差异有统计学意义(P<0.05);腹膜透析患者血清NGAL水平与CRP、TNF-α、IL-6等指标均有正相关性,差异有统计学意义(P<0.05)。强化透析方案后患者Kt/V比值升高,腹膜透析充分性提高,血清NGAL、CRP、IL-6、TNF-α水平均降低,差异有统计学意义(P<0.05)。结论:在腹膜透析患者中,血清NGAL与CRP、TNF-α、IL-6有不同程度的正相关。强化透析方案可提升腹膜透析充分性并改善腹膜透析患者微炎症状态。
Objective: To study the correlations between the level of serum neutrophil gelatinase-associated lipocalin (NGAL) and inflam- matory factors and assess the effects of strengthened peritoneal dialysis to find a new way to ameliorate mieroinflammation in peritoneal dialysis patients. Methods: Eighty peritoneal dialysis patients and 60 healthy people for the eontrnl group were enrolled in Ibis study. The clinical and biochemical data of those subjects were collected and the serum NGAL, CRP, TN F-a, IL-6 were measured and compared. The correlations between serum NGAL and inflammatory factors in peritoneal dialysis patients were analyzed. Changes of the serum NGAL, CRP, TNF-a,IL-6 and Kt/V ratio through strengthened peritoneal dialysis wel~ obserw^d. Results: The serum NGAL was signit^cantly higher in peritoneal dialysis patients (655.22 ug/mL+103.8 ng/mL) than that in the control group (49.14 ng/mL+-7.20 ng/mL)(P〈0.05 ). Serum NGAL in peritoneal dialysis patients was positively correlated with CRP,TNF-~,IL-6. Kt/V ratio increased while the levels of serum NGAL, CRP,TNF--~, IL^5 decreased through strengthened peritoneal dialysis. And the differences were statistically significant (P〈0.05). Conclusion: Serum NGAL in peritoneal dialysis patients is positively correlated with CRP, TNF-cdL-6. Strengthened peritoneal dialysis may enhance dialysis adequacy, and ameliorate the microinflammation in peritoneal dialysis patients.
