摘要
目的:探讨人工肱骨头置换治疗老年肱骨外科颈粉碎性骨折的疗效。方法:采用Thompson入路,人工肱骨头置换治疗老年肱骨外科颈粉碎性骨折49例,按Neer分型:三部分骨折28例,四部分骨折21例。术后随访,行Neer肩关节功能评分、VAS疼痛评分、肩关节活动度测定。结果:患者均无感染、假体周围骨折、无假体松动、脱位以及神经损伤等并发症。按照Neer肩关节功能评分标准提示优良率90.13%,VAS疼痛评分为15.25±62分。肩关节活动度上举为100.5±5.2,后伸41±8.2,外展88.2±10.1,外旋72±6.1,内旋75.2±2.9.结论:人工肱骨头置换术是治疗老年肱骨外科颈粉碎性骨折有效的方法,术后功能恢复快,能有效减轻肩关节疼痛,恢复关节活动度,疗效满意。
Objective：To investigate the efficacy of replacement of humeral head prosthesis for the comminuted fracture of old age surgical neck of humerus. Methods：Through thompson approach, 49 cases of comminuted fracture of old age Surgical neck of humerus were treated with replacement of humeral head prosthesis. According to Neer classification, there were 28 cases of three-part fractures and 21 cases of four-part fractures. In postoperative follow-up, we do Neer shoulder function score, VAS pain scores, and shoulder activity was measured. Results： The patients had no infection, periprosthetic fracture, prosthesis loosening, dislocation, or nerve damage and other complications. According to the Neer shoulder function score, the excellent and good rate was 90.13% ,and VAS pain score was 15.25 ± 62. The shoulder joint activity： lifting 100.5 ± 5.2 , abduction 41 ± 8.2 , extension 88.2 ± 10.1 , external rotation 72± 6.1 and internal rotation 75.2±2.9 . Conclusion： Replacement of humeral head prosthesis is an effective method for the comminuted fracture of old age Surgical neck of humerus. It leads to fast functional recovery, and can effectively alleviate the shoulder pain, restore range of motion, and the results were satisfactory.
