摘要
目的探讨动态心电图与常规心电图诊断冠心病的临床效果。方法将浙江省衢州市衢化医院2009年4月～2011年4月收治的60例冠心病患者同时进行动态心电图与常规心电图诊断，观察两种方法心肌缺血检出率和心律失常检查情况。结果规心电图检查患者心肌缺血阳性有29例，检出率为48．3％；动态心电图检查患者心肌缺血阳性有49例，检出率为81．7％。两种方法检查的心肌缺血阳性检出率比较，差异有统计学意义（χ2=7．23，P〈0．05）。动态心电图检查室性早搏二／三联、室性早搏成对、房性早搏二／三联、房性早搏成对、短阵室上速的检出率均明显的高于常规心电图检查室性早搏二／三联、室性早搏成对、房性早搏二／三联、房性早搏成对、短阵室上速的检出率，两种检查心律失常的检出率比较，差异有统计学意义（χ2=7．35，χ2=7．28，χ2=7．24，χ2=7．59，χ2＝7．18，P〈0．05）。而两种检查的房室传导阻滞和房性早搏早发的检出率比较，差异无统计学意义（χ2=0．26，χ2=0．17，P〉0．05）。结论临床中对于冠心病患者采取动态心电图的诊断效果优于常规心电图的诊断效果，且属于无创检查，重复性高，对冠心病心肌缺血和心律失常的检查具有重要的意义。
Objective To investigate the Hoher and routine ECG diagnosis of coronary heart disease with clinical effect. Methods 60 cases of coronary heart disease among patients with simultaneous Holter and routine ECG diagnosis were collect- ed in Quhua Hospital in Quzhou City form April 2009 to April 2011, and two methods of myocardial ischemia and arrhythmia detection rate check situation were observed. Results Regulation of ECG positive for myocardial ischemia in patients with 29 cases, the detection rate was 48.3%; Holter positive for myocardial ischemia in patients with 49 cases, the detection rate was 81.7%. Compared with two ways to check the positive rate of myocardial ischemia, there was a significant difference （χ2 = 7.23, P 〈 0.05）. Hoher premature ventricular contractions two/triple, paired premature ventricular, atrial premature beats two/ triple, atrial premature beats in pairs, paroxysmal supraventrlcular tachycardia detection rate was significantly higher than the conventional ECG ventricular premature beats two/triple, paired premature ventricular contractions, atrial premature beats two/ triple, atrial premature beats in pairs, paroxysmal supraventricular tachycardia, compared with detection rate of the two checks arrhythmia, there were significant differences （χ2 = 7.35, χ2 = 7.28, χ2 = 7.24, χ2 = 7.59,χ2 = 7.18, P 〈 0.05）. The two kinds of checks atrioventricular block and atrial premature premature detection rate showed no significant difference （χ2 = 0.26, χ2 = 0.17, P 〉 0.05）. Conclusion For clinical coronary heart disease patients taking Holter and routine ECG diagnosis is better than conventional diagnostic results, and are noninvasive, high repeatability, coronary heart disease myocardial ischemia and arrhythmia examination has important significance.
