摘要
目的了解本地区临床分离志贺菌中超广谱β-内酰胺酶(ESBLs)的基因型及其耐药特点。方法收集2008年-2011年本地区分离志贺菌83株,PCR检测基因型,阳性产物在GenBank上进行比对确定亚型,按照Kirby-Bauer法检测志贺菌对8种头孢类抗生素耐药情况。结果 83株志贺菌共检出携带ESBLs菌株33株,占39.76%。耐药基因型以OXA和CTX-M-9型为主,并伴有TEM型和SHV型。DNA序列分析证实17株为OXA-1亚型(52.52%),11株为CTX-M-14亚型(33.33%),5株志贺菌检出同时携带2种耐药基因。药敏结果显示产ESBLs菌株对头孢唑林、头孢噻肟和氨曲南有较高耐药性,耐药率分别为54.55%、48.48%和21.21%。携带2种耐药基因菌株对头孢类抗生素耐药性明显增强。结论本地区志贺菌携带ESBLs耐药基因水平较高,以OXA-1亚型和CTX-M-14亚型为主,大部分产ESBLs菌株呈多重耐药,并随携带耐药基因种类增多,其耐药性增强。
Objective To understand the genotype and drug resistance characteristics of extended spectrum beta - lactamases (ESBLs) in clinical isolates of ShigeUa in Xicheng District of Beijing. Methods Eighty - three chnical isolates of Shigella were colleted from 2008 to 2011 to detect the genotype by polymerase chain reaction( PCR), then the PCR positive productions were sequenced and compared in GenBank to determine gene subtypes. The drug susceptibility of Shigella to 8 kinds of cephalosporanic antibiotics were performed by Kirby - Bauer method. Results Among 83 strains of Shigella, 33 strains carried ESBLs gene(39.76% ). The main genotypes were OXA and CTX - M - 9, and accompanied with TEM - type and SHV - type. Nucleotide sequence analysis showed that 17 strains (52.52%) were OXA- 1 subtypes, 11 strains(33.33% ) were CTX- M - 14 subtypes, 5 strains carried 2 kinds of ESBLs drug resistance genes. The results ef drug susceptibility test showed that the ESBLs -producing strains were higher resistant to Cefazolin, Cefotaxime and Aztreonam and the resistant rates were 54.55%, 48.48% and 21.21% respectively. The antibiotic resistance increased obviously in the strains carrying 2 kinds of drug resistance genes. Conclusion The Shigella had high carrier rate of ESBLs drug resistance genes in Xicheng distict. The main genotypes are OXA- 1 subtype and CTX- M -14 subtype. The most ESBLs -producing strains show multidrug resistance and the drug resistance increased with the increase of drug resistance genes.
