摘要
目的:探讨Caspase3、Bcl-2的表达在结肠癌诊断中的临床意义.方法:通过采用免疫组织化学S P检测Caspase3、Bcl-2蛋白在结肠癌、结肠腺瘤和正常结肠黏膜组织中的表达情况,分析其阳性表达及强度与临床病理特征之间的联系.结果:在结肠腺癌、腺瘤与正常黏膜中Caspase3蛋白的阳性表达率依次升高.Bcl-2蛋白在正常结肠黏膜、腺癌、腺瘤中的阳性表达率依次升高.与正常黏膜相比,Caspase3蛋白在腺癌、腺瘤中的阳性表达率均降低明显,差异有统计学意义(50.0%,48.5%vs90.0%,均P<0.05),Bcl-2蛋白在腺癌、腺瘤中的阳性表达率均升高明显,差异有统计学意义(83.4%,66.7%vs 30.0%,均P<0.05).Caspase3、Bcl-2蛋白阳性表达率均与肿瘤分化程度密切相关(P<0.05),且随着肿瘤分化程度的降低,Caspase3、Bcl-2阳性表达率逐渐降低(P<0.05),与其他临床病理特征差异无统计学意义,且Caspase3和Bcl-2呈现负相关(r=-0.320,P<0.05).结论:Bcl-2作为是判断结肠癌恶性程度的参考指标之一,其可能通过抑制Caspase3激活抑制细胞凋亡,延长其生存时间,增加不稳定性基因,导致细胞凋亡异常及增生过度.
AIM: To detect the expression of Caspase 3 and Bcl-2 in colon cancer and to analyze their diagnostic significance.
METHODS: Immunohistochemistry was used to detect the expression of Caspase 3 and Bcl-2 proteins in colon cancer, colon adenoma and normal colonic mucosa. The relationship between Caspase 3 and Bcl-2 protein expression with clinicopathological features of colon cancer was analyzed.
RESULTS: Compared with the normal colonic mucosa, the positive rate of Caspase 3 protein expression significantly declined in adenocarcinoma and adenoma (90.0% vs 50.0%, 48.5%, both P 〈 0.05) and that of Bcl-2 protein expression significantly increased (30.0% vs 83.4%, 66.7%, both P 〈 0.05) in adenocarcinoma and adenoma. Caspase 3 and Bcl-2 protein expression was significantly related to the degree of tumor differentiation (both P 〈 0.05) but not to other clinicopathological features. Expression of Caspase 3 was negatively correlated with that of Bcl-2 (r = -0.320, P 〈 0.05).
CONCLUSION: Bcl-2 can be used as a marker for evaluation of the malignancy of colon cancer. Bcl-2 functions in colon cancer possibly by restraining Caspase 3 activation.
