百日咳综合征79例临床特征分析 被引量：17

摘要 目的了解百日咳综合征临床特点,探讨其与百日咳的鉴别诊断及防治。方法以2011年1—6月重庆医科大学附属儿童医院呼吸科收治的63例百日咳综合征患儿为2011年组;以2010年1—6月收治的16例百日咳综合征患儿为2010年组,比较其临床特点、实验室检查、影像学检查及治疗。结果 (1)两组患儿发病均具有明显季节及年龄特点,第二季度患病例数高于第一季度,且年龄以<6个月者多见;(2)两组患儿均有阵发痉挛性咳嗽;2011年组有感染中毒症状者及并发症者较2010年组明显增加,差异有统计学意义(P<0.01)。(3)患儿白细胞总数大多增高,且淋巴细胞比例高于中性粒细胞;(4)2011年组的胸部影像学表现类型较2011年组复杂;(5)2011年组治疗困难,平均住院日增加。结论百日咳综合征患儿具有显著的年龄及季节特点,2011年患病例数明显增加;百日咳综合征与百日咳临床特点相似;亟需灵敏、特异的百日咳杆菌实验室诊断方法用于临床。
