摘要
目的探讨腹腔镜术与传统手术在治疗小儿腹股沟斜疝方面的疗效及优缺点。方法选择2011年1月～2013年6月仙桃市第一人民医院普外科治疗的腹股沟斜疝的患儿200例，按随机分组的方法，分为腹腔镜组和传统手术组，每组各100例。分别采用腹腔镜和传统手术方式行疝囊高位结扎术，比较两组各项手术指标以及术后并发症情况。结果①腹腔镜组患者术中出血量[（12．9±6．8）mL]少于传统手术组[（37．2±2．0）mL]，手术时间、住院时间、切口长度[（27．3±9．4）min、（4．5±1．1）d、（1．3±0．2）cm]短于传统手术组[（45．2±17．6）min、（7．6±1．5）d、（2．5±0．4）cm]，差异均有统计学意义（P〈0．05）。②腹腔镜组患者术后并发症发生率为7％（7／100），传统手术组并发症发生率为20％（20／100），差异有统计学意义（P〈0．05）。结论腹腔镜下小儿疝囊高位结扎术，具有操作简单、创伤小、术后恢复快、术后恢复好、对精索损伤小等优点，是小儿疝气治疗的微创术式，并且深受患儿及家长的欢迎。
Objective To study the clinical effect of laparoscopic surgery and traditional surgery in treating pediatric inguinal hernia. Methods From January 2011 to June 2013 in Department of General Surgery, the First People＇s Hospi- tal in Xiantao City, 200 cases of pediatric inguinal hernia were selected and divided into laparoscopic treatment group and traditional surgical treatment group randomly with 100 cases in each group. The hernia sac high ligation operation were carried out with laparoscope and the traditional surgery. The operation index and postoperative complications of two groups were compared. Results ①Intraoperatve blood loss in laparoscopic treatment group [（12.9±6.8） mL] was less than that in traditional surgical treatment group [（37.2±2.0） mL]; surgery time, hospital stays, incision length in laparo- scopic treatment group [（27.3±9.4） min, （4.5 ±1.1） d, （1.3±0.2） cm] were shorter than those in traditional surgical treat- ment group [（45.2±17.6） rain, （7.6±1.5） d, （2.5±0.4） cm], the differences were statistically significant （P 〈 0.05）. ②Occurrence rate of postoperative complications in laparoscopic treatment group was 7% （7/100）, occurrence rate of postop- erative complications in traditional surgical treatment group was 20% （20/100）, the difference was statistically signifi- cant （P 〈 0.05）. Conclusion Laparoscopie pediatric hernia sac high ligation has the advantage of simple operating method, small trauma, quick recovery, good postoperative recovery, small injuries of spermatic cord, it is minimally in- vasive surgery of pediatric hernia treatment and popular among children and their parents.
