摘要
目的探讨护理干预对行主动脉内球囊反搏术(IABP)的急性心肌梗死(AMI)患者康复效果的影响。方法观察分析20例AMI患者IABP治疗前后的心率、收缩压、舒张压、平均动脉压、射血分数、心脏指数和尿量数据,对患者进行术前、术中、术后护理干预,评价预后的改善情况。结果 20例患者均成功完成治疗,术后2例发生出血或局部血肿并发症;16例病情好转,转普通病房继续治疗;4例并发多器官功能衰竭死亡。相关监测指标显示,治疗前后患者心率、收缩压、舒张压、平均动脉压、射血分数、心脏指数、尿量的差异均有统计学意义。结论 护理干预在对AMI患者进行IABP的治疗中起着重要作用,能明显促进病情康复。
ObjectiveTo explore the influence of nursing interventions on rehabilitation effect of acute myocardial infarction （AMI） patients treated with intra- aortic balloon counterpulsation（ I- ABP）. Methods Clinical indexes such as heart rate, systolic pressure, diastolic pressure, mean arterial pressure, ejection fraction, cardiac index and urinary production before and after implemen- tation of IABP in 20 AMI patients were observed and analyzed. 20 patients were conducted with preoperative, intraoperative and postoperative nursing interventions, and the prognosis was as- sessed. Results 20 patients successfully completed treatment, and 2 patients had postoperative hemorrhage or hematoma; 16 patients turned well and were moved to general wards for further treatment; 4 patients died of multiple organ failure. There were significant differences of HR（ t = 4.61, P〈0. 0001）, systolic pressure （t = 4.12, P = 0. 0002）, diastolic pressure （t = 4.27, P = 0.01）, mean arterial pressure （ t = 3.23, P = 0. 0026）, EF（ t = 5.25, P〈0. 0001）, cardiac in- dex（ t = 8.00, P〈0. 0001）and urinary production（ t = 2.26, P = 0. 028） before and after IABP treatment in all the patients. Conclusion Nursing intervention plays an important role in the treatment of AMI patients with IABP, which can significantly promote the rehabilitation of pa- tients.
