摘要
糖尿病肾病为糖尿病的常见并发症,中医治疗突显其优势,笔者将1型糖尿病和2型糖尿病导致的糖尿病肾病,分别"脾瘅肾病"和"消瘅肾病"。并指出二者在发展过程中的病机不同,脾瘅肾病是由郁、热、虚导致的络损;消瘅肾病是以气阴两虚为基础导致的络损,二者在早期当区别治疗,发展到后期又有相同的病机和表现。总言之,糖尿病肾病以虚、瘀、浊为核心病机,以虚实同治,标本兼顾为治则,以补虚、活血通络、温阳泻浊为基本治法,又根据虚、瘀、浊的偏重而分别处之,选用黄芪建中汤、当归补血汤、抵挡汤、真武汤等有效方剂。
Diabetic nephropathy is a common complication of diabetes, Chinese medicine treatment has its advantages, the authors distinguish the diabetic nephropathy which due to type 1 diabetes or type 2 diabetes, respectively spleen hate nephropathy and consumer hate nephropathy. And the pathogenesis of them in the development process is different, the spleen hate nephropathy dues to stagnation, hot, and weakness which lead to vascular injury; the consumer hate nephropathy results from deficiency of qi and yin. In the early days, both of them have different pathogenesis and performance, so they should have different treatment, however, to the late stage they have the same pathogenesis. In short, diabetic nephropathy, the core pathogenesis is deficiency, stasis, damage; principles of treatment are both specimens and virtual treatment, treatments are complement deficiency, activating blood circulation, wenyang xiezhuo; prescriptions are from Huangqijianzhong Decoction, Dangguibuxue Decoction, Didang Decoction, Zhenwu Decoction and so on.
