摘要
目的探讨血清中破骨细胞分化因子(Osteoclast differentiation factor,ODF)和破骨细胞生成抑制因子(Osteoclastogenesis Ihibititory Factor,OCIF)及血清钙和碱性磷酸酶在肺癌骨转移中的诊断价值。方法选择经病理学确诊的肺癌患者186例,分为骨转移组104例,无骨转移组82例,以健康成年人30例和肺部其它疾病66例作为对照组,采用ELISA法检测血清中ODF和OCIF水平,偶氮砷Ⅲ法测定血清钙,速率法测定血清碱性磷酸酶水平。结果肺癌骨转移组患者血清ODF和OCIF分别为(32.22±6.22)ng/L、(41.23±8.13)ng/L,明显高于非骨转移组的(8.35±5.42)ng/L、(10.15±4.42)ng/L,具有较高临床诊断价值;ODF诊断肺癌骨转移的灵敏度、特异度分别为90.38%、86.59%;OCIF诊断肺癌骨转移的灵敏度、特异度分别为86.54%、84.15%;在104例肺癌骨转移患者中有38例血清碱性磷酸酶升高,发生率为36.54%,对肺癌骨转移有辅助诊断价值。结论肺癌患者发生骨转移时,血清中ODF和OCIF水平显著增高,与血清碱性磷酸酶联合检测,可作为肺癌患者判断骨转移的重要参考指标,血清钙检测对判断肺癌骨转移临床意义不大。
[Abstract] Objective To investigate the value of osteoclast differentiation factor (ODF), osteoclastogenesis ihibi- titory factor (OCIF), calcium and alkaline phosphatase (ALP) detection for clinical diagnosis and condition evaluation in lung cancer bone metastasis. Methods 186 data of patients with lung cancer were analyzed. The data included 87 patients without bone metastasis and 104 bone metastasis. The concentration of serum ODF and OCIF in each group were detected by ELISA. The concentration of serum calcium and ATP were detected by Calcium Arsenazo III method and rate method respectively. Results The concentration of ODF and OCIF in bone metastasis group were (32.22 ± 6.22) ng/L and (41. 23±8.13) ng/L respectively. The concentration of ODF and OCIF of non-bone metastasis group were (8. 35± 5.42) ng/L and (10. 15±4. 42) ng/L. The area under the ROC curves of ODF and OCIF diagnosed bone metastasis of lung cancer were 0.91 and 0.87, which indicated a well diagnostic value. The sensitivity and specificity of ODF and OCIF in diagnosing lung cancer with bone metastasis were 90. 38% and 86.59%, and 86.54% and 84. 15%, respectively. In the 104 cases only 10 were detected an increasing of calcium level (9.62%) but 38 had an increased serum ALP level that accounted for 36.54QA. Conclusion When bone metastasis occurs in lung cancer patients, the concentration of serum ODF and OCIF will significantly increase. Detection of serum ODF,OCIF and ALP would be an reference index which can monitor bone metastases and evaluate patient's condition.
