摘要
目的 :探讨 PCNA、Cyclin D1 、H- ras在胃癌中的表达规律与临床意义。方法 :运用 SP免疫组织化学技术对 6 8例胃癌标本进行检测。结果 :胃癌 PCNA L I平均为 6 1.88± 16 .0 0 ,显著高于正常粘膜 ,良性溃疡和癌旁粘膜。 Cyclin D1 在胃癌中的阳性率为 80 .88% ,Cyclin D1 高表达与淋巴结转移呈正相关 ;H- ras在胃癌阳性表达率为73.5 3% ,与 Cyclin D1 表达相似 ,H- ras过表达与淋巴结转移呈正相关 ;H- ras与 Cyclin D1 之间显示出正相关 ,P值<0 .0 0 1;PCNA与预后关系密切 (P<0 .0 0 1) ,PCNA L I越高 ,术后生存时间越短 ,预后越差。结论 :(1) PCNA是反映细胞增殖状态的良好指标 ,PCNA L I对估计胃癌恶性程度 ,判断预后及指导临床治疗有重要价值 ;(2 ) H- ras,CyclinD1 过表达是胃癌发生的早期变化 。
Objective:The expression of PCNA,cyclin D 1,and H-ras in gastric carcinoma were detected to determine the characteristics of the expression of them during gastric carcinomagenesis and progression. Methods:Immunohistochemical assays were used in a series of 68 gastric carcinomas .Results:(1)The means PCNA labelling index(PCNA LI) in 68 gastric cancers was 61.88±16.00,which was significantly higher than that in normal mucosals,benign ulcer and paracancerous mucosal.(2)Immunostaining cyclin D 1 was detected in 55 gastric carcinomas of 68(80.88%). The overexpression of cyclin D 1 was closely correlated with lymph node metastasis(P<0.05).(3)73.53%(50/68) was stained positive as for H-ras.The correlation of H-ras overexpression lymgh node involvement(P<0.05) was detected. (4)The expression of H-ras and cyclin D 1,H-ras was closely correlated .(5)Survival analysis showed that PCNA LIwas closely correlated with the survival time(P<0.001).Conclusions:(1)PCNA is an excellent parametre which shows cell proliferation.PCNA LI can be used as a biomarker in assessing the malignance of gastric cancer, judging the prognosis and directing clinical therapy.(2)The overexpressions of H- ras and cyclin D 1 are early molecular changes in stomach carcinomagenesis,which are condusive to early diagnosis of gastric cancer.
