摘要
目的:探索早期糖尿病肾病(DN)的危险因素,为临床防治早期DN提供参考依据。方法:多中心收集早期DN 182例(A组),以170例单纯糖尿病(DM,B组)为对照,收集年龄、病程、血压及腰臀比(WHR),检测血脂、糖化血红蛋白(HbA1c)及尿白蛋白(uAlb),计算尿白蛋白排泄率(UAER),进行统计学分析。结果:(1)两组比较:病程、SBP、DBP、WHR、HbA1c、TG、HDL、UAER差异有统计学意义(P<0.05,P<0.01),LDL、TC差异无统计学意义(P>0.05)。(2)以UAER为因变量,其他指标为自变量,进行logistic分析。其中病程、SBP、WHR、HbA1c先后被列入方程,为早期糖尿病肾病的独立危险因素(P<0.01)。(3)spearman相关分析显示:HbA1c与病程、TG,WHR与TG、HbA1c呈正相关(P<0.05)。结论:糖尿病病程、收缩压、腰臀比、糖化血红蛋白是早期DN的独立危险因素,严格控制血糖血压、减肥有利于延缓糖尿病肾病的进展。
Objective ：To explore the main risk factors related in early stage diabetic nephropathy（DN） and provide the ba- sis for clinical treatment and prevention. Methods ：182 patients multicentric with early DN were selected as observation group（ group A） ,while 170 patients with the Type 2 diabetes meUitus and also without diabetic complications （T2DM, group B） were took as the control. The indexes in serum or urine were detected, which were glycosylated hemoglobin Ale （HbAle）,Triglyceride （TG）, total cholesterol（TC） ,low density lipoprotein（LDL） ,high density lipoprotein（HDL） ,urinary albumin excretion rate （UAER）, all patients ~ clinical materials were collected, including age, duration of disease, waist - to - hip ratio （ WHR）, systolic and diastolic pressure, the levels of biological indexes were analyzed by the statistical method. Results： （ 1 ） There was statistical significance in all the indexes a- bove between group A and group B except age,TC and LDL（P 〈0. 05 ,P 〈0.01 ） ; （2） Logistic analysis showed that the main risk factors of early diabetic nephropathy were duration of diabetes, WHR, HbA1 c, systolic pressure （ P 〈 0.01 ）. （3） Spearman analysis showed WHR positively correlated with TG and HbA1 c（P 〈 0.05 ） ;Positive correlation were also found between HbA1 c and TG,duration of disease（P 〈 0.05 ）. Conclusion：Duration of diabetes, WHR, HbAlc, systolic pressure were the main risk factors in early DN. Control of hyperglycemia,blood pressure and weight loss can delay the development of diabetic nephropathy.
