双血管融合重建方法评估脑动静脉畸形的价值 被引量:7

Value of dual volume reconstruction from 3-D DSA to evaluating angioarchitecture of cerebral arteriovenous malformation

摘要 目的探讨双容积重建技术进行颅内双血管融合重建方法评估脑动静脉畸形的价值。方法对10例含两组以上动脉供血的脑动静脉畸形患者均采用选择性全脑血管造影及三维血管重建,将获得的两组三维重建供血动脉(一组颈内动脉和一组椎动脉或两组颈内动脉)的数据用双容积重建技术重建,将两组动脉的三维血管像融合。结果在同一图像中可以获得两组动脉系统血管的融合像,解剖关系的准确性良好;可以显示脑动静脉畸形患者血管巢供血来自不同的动脉系统,血管巢内有一部分接受两组动脉同时供血。结论采用双血管融合技术在DSA图像上显示脑动静脉畸形患者血管巢的全貌,并能分别显示血管巢的不同供血来源及其内部的沟通,更有利于认识脑动静脉畸形的内部构筑、供血来源等。 Objective To investigate the value of dual volume reconstruction (DVR) from three dimensional digital subtraction angiography (3-D DSA) to evaluating the angioarchiteeture of cerebral arteriovenous malformation (CAVM). Methods Ten patients with high flow CAVM were prospectively recruited in this study. The 3-D DSA with the imaging mode of 5sDSA was performed in all the patients. The data of original DSA images were then transferred to a workstation (Siemens x-workplace). The image post-processing included four steps. Firstly, the imaging data of two groups of arterial system including bilateral internal carotid arteries, bilateral vertebral arteries or one side of internal carotid artery combined with one vertebral artery were selected at the same time. Secondly, two different kinds of pseudo color were assigned to the two arterial systems such as one with red and the other one with white. Thirdly, the selected imaging data were processed using the technique of DVR to show the angioarchitecture of CAVM. Fourthly, the imaging data of one arterial system were transformed into computed tomography angiography (CTA) imaging which could show the vessels and the skull bones and the imaging data of the other one arterial system did not. Results The angioarchitecture of CAVM including feeding arteries and draining veins could be clearly seen in the reconstructed images. The reconstructed CTA images could clearly showed the position relation of the CAVM and the marks of skull bones, which was helpful to guiding the resection of the CAVM. Conclusions The images reconstructed using DVR can clearly show the angioarchitecture of CAVM and can provide guidance for resection of CAVM.
