摘要
目的:分析导致产后出血的原因和相关危险因素,从而有针对性地防治产后出血,降低孕产妇死亡率。方法:回顾性分析攀枝花市妇幼保健院2009年1月至2011年12月发生的产后出血135例产妇的临床资料。结果:产后出血率为3.45%(135/3917);宫缩乏力(55.56%)和胎盘因素(37.78%)是主要的产后出血原因;剖宫产的产后出血发生率高于阴道分娩;产后出血与分娩年龄、孕次(≥3次)、产次、分娩孕周、妊娠合并症及并发症、多胎妊娠、新生儿体重有关(P<0.01;P<0.05)。结论:加强产前宣教及孕期系统保健,筛查高危因素,加强高危妊娠的管理,减少社会因素剖宫产,以降低因产后出血引起的合并症及孕产妇死亡率。
Objective：To explore the reasons and related risk factors for postpartum hemorrhage and to provide basis for preventing postpartum hemorrhage and decreasing maternal mortality. Methods：The clinical data of 135 cases of the parturient who underwent postpartum hemorrhage from January,2009 to December, 2011 were analyzed retrospectively. Results ：The rate of postpartum hemorrhage was 3. 45% （135/3917）.Main causes for postpartum hemorrhage were uterine inertia （55.56%） and placental related factors （37.78%）. The incidence of postpartum hemorrhage for cesarean section was higher than that of vaginal delivery. The risk factors significantly related to postpartum hemorrhage were as follows： the elderly pregnant mother,the times of the mother for pregnant （ I〉 three times）, the times of the mother for giving birth to a child,the fetal weight and the pregnancy complication of the mother（ P 〈 0.01 ;P 〈 0.05）. Conclusions：To reduce the mortality rate for the pregnant and parturient caused by the postpartum hemorrhage, some meas- ures should be done which including systematic health instruction on prenatal period and pregnant period,the screen for the risk factors,further management for the high-risk pregnancy, reduction in the number of social factor cesarean section.
