摘要
目的对急性颅脑外伤后凝血功能及D-二聚体的变化进行临床观察。方法以2012—2013年在我院就诊的180例急性颅脑外伤患者(实验组)和150例健康体检者(对照组)作为研究对象,分别对其凝血功能和D-二聚体水平进行检测,并对结果进行比较。结果实验组中GOSⅠ级患者的TT、Fbg值与其余各级相比TT明显延长,而Fbg水平则明显降低,差异有统计学意义(P<0.05);入院时D-D含量≥12.7mg/L患者均死亡,含量越高GOS评级越低;实验组患者TT、PT、Fbg和D-D与对照组相比均表现出受伤后迅速延长或升高的趋势(P<0.05),逐日下降,伤后7d与对照组水平接近;继发性外伤性脑梗死患者D-D含量增多(P<0.01),PT明显缩短(P<0.05),Fbg降低(P<0.05),D-D含量增多(P<0.01),APTT和TT差异无统计学意义(P>0.05);迟发性颅内血肿患者则出现PT明显延长(P<0.01),D-D含量增多(P<0.01),APTT缩短(P<0.05),Fbg升高(P<0.05)。结论凝血指标与D-D对急性颅脑损伤的程度、预后及并发症有一定关系。
Objective To conduct the clinical observation of coagulation and D-dimer changes after the acute brain injury. Methods Totally 180 cases of acute brain injory patients (experimental group) and 150 cases of healthy examination patients (control group) in our hospital from 2012 to 2013 were selected as the study objects, and their coagulation and D-dimer levels were detected respectively, then the results were compared and analyzed. Results In the GOS ]- patients of the experimental group, compared with other levels, TT was significantly prolonged, while Fbg level significantly declined, with significant difference (P^0.05) and statistical significance. The patients admitted in hospital whose D-direct content ~12.7 mg/L were all died, and the more the content, the lower the COS score. Compared with the control group, TT, PT, Fbg and D-dimer of the experimental group showed the trend of extending or increasing rapidly after injury, with significant difference (P〈0.05), subsequently daily declined, 7d after injury showed a level close to the control group. The traumatic cerebral infarction secondary to appear D-Dimer levels increased (P〈0.01), PT significantly reduced (P〈0.05), Fbg lower (P〈0.05), D-direct levels increased (P〈0.01), APTT and TT was not significantly different (P〉0.05) ; delayed intracranial hematoma occurs PT was significantly prolonged in patients (P〈0.01), D-Direct levels increased (P〈0.01), APTT shortened (P〈0.05), Fbg increased (P〈0.05). Conclusion There is a certain relationship between the coagulation, D-dimer and the level, prognosis, complications of acute brain.
