阿尔茨海默病和血管性痴呆患者海马杏仁核体积的 MRI对比研究 被引量:25

摘要 目的 探讨海马、杏仁核在 Alzheimer病 (AD)和血管性痴呆 (VD)发病中的作用。方法 AD患者 15例, VD患者 7例,正常老年对照 14例。行头颅 MRI测量海马杏仁核体积。应用标准化体积比较 AD、 VD组与正常组的差异,分析 VD患者海马杏仁核萎缩与临床症状的关系。结果 AD患者海马杏仁核体积比对照组萎缩 25% (P< 0.05)。 VD患者萎缩 12%~ 18%,但与 AD和对照组比均无显著性意义。右侧海马杏仁核体积联合区分 AD的敏感性为 86.67%,特异性为 85.71%,准确率为 86.19% (P< 0.05)。临床分析显示, VD患者病前有记忆力减退者,海马杏仁核萎缩严重。结论海马杏仁核结构萎缩是 AD和 VD的重要病理学基础,缺血加重老化损伤是 VD发病的可能机制,海马杏仁核结构以外的某些脑组织结构在 VD发病中可能同样具有重要作用。 Objective To explore troles of Hp and AN in the pathogenesis of AD andVD.Method The volume of HP and AN were measured and normalized by the products of three distances of the cranial:anterior posterior,superior inferior,left right,The relationship bvetween HP and AN atrophy and clinical symdrome was analysised in VD patients.Results The volumes of HP and AN were declined 25% in AD patients ,compared to normal controls (P< 0.05), while those of VD decreased 12%~ 18%, but no significant difference were found when compared to AD or normal controls,The sensitivity and specificity of hippocampal and amygdala volumetric measures in Discriminant Function Analysis between Alzheimer patients and controls were 86.67% and 85.71% ,respectively.Clinical data of VD patients showed that the fHP and AN atrophy were more serious in those with a memory complaint before the diagnosis of VD.Conclusion The HP and AN atrophy was the important pathological basis for the development of AD and VD.The possible pathogenesis of VD include ischamic overlap aging and other brain tissues damage apart from HP and AN.
