2种全身麻醉用于老年腹腔镜胆囊手术的比较 被引量:4

摘要 目的:比较依托咪酯与丙泊酚复合瑞芬太尼应用于老年患者静脉全身麻醉下腹腔镜胆囊切除术的麻醉效果及不良反应。方法:选择我院90例行静脉全身麻醉下腹腔镜胆囊切除术老年患者按随机数字表分为2组,Ⅰ组45例,Ⅱ组45例。Ⅰ组实施瑞芬太尼、依托咪酯全凭静脉麻醉,Ⅱ组实施瑞芬太尼、丙泊酚全凭静脉麻醉。观察2组患者术中血流动力学指标,记录苏醒时间与拔管时间,观察不良反应发生情况。结果:Ⅱ组患者麻醉诱导后(T1)SBP、DBP均较诱导前(T0)明显降低,均具有显著性差异(P<0.05),但Ⅰ组没有明显变化;Ⅱ组麻醉诱导后(T1)DBP明显高于Ⅰ组,具有显著性差异(P<0.05),Ⅰ组和Ⅱ组气管插管即刻(T2)、气管插管后3 min(T3)、腹腔镜置入即刻(T4)、腹腔镜置入3 min(T5)、气管拔管即刻(T6)、气管拔管后3 min(T7)与诱导前(T0)比较均无明显改变,Ⅰ组和Ⅱ组组间各时点亦无明显差异;Ⅰ组和Ⅱ组的苏醒时间分别为16.16±5.86 min和10.84±4.06 min,Ⅰ组和Ⅱ组的气管拔管时间分别为18.91±6.23 min和14.40±5.38 min,2组具有显著性差异。结论:施瑞芬太尼联合依托咪酯全凭静脉麻醉用于老年腹腔镜胆囊切除术具有麻醉诱导平稳、镇痛效能高、术中可控性好的优势,但瑞芬太尼联合丙泊酚全凭静脉麻醉方法的苏醒时间及拔管时间更短及不良反应发生率更低,临床应根据需要合理选择麻醉方法。
