摘要
目的探讨血清脑钠肽(BNP)和心肌钙蛋白T(cTnT)的浓度变化,在急性冠脉综合征(ACS)临床诊疗策略制定及预后评估中的意义。方法200例ACS患者按冠脉造影的病变Gensini积分不同分为Gensini〉40、20~40分和〈20分3组,对比分析不同组别的BNP及cTnT水平差异及二者关系。结果cTnT水平均为Gensini积分〉40分组〉20~40分组〉〈20分组,组间差异有显著性(P〈0.000~0.021);BNP水平除20~40分组和〈20分组无显著差异外(P=0.059),其与〉40分组间比较均有显著差异(P〈0.000~0.035);Gensini积分与BNP和cTnT水平均呈显著正相关(分别r=0.288和r=0.354,P〈0.001)。结论ACS患者血清BNP及cTnT水平变化,可反映心肌坏死及心功能变化程度,对临床诊疗及预后评估可能有一定参考价值。
Objective To explore the changes of brain natriuretic peptide and cardiac troponin T in patients with different severity and extent of coronary artery lesion and risk stratification of acute coronary syndrome and discuss the significance of BNP and cTnT level of ACS in making therapeutic strate in clinic and judging prognosis. Methods The serum BNP and cTnT level was determined in 200 consecutive ACS patients. The 200 ACS patients were further divided into three groups (Gensini scores:〉40scores, 20-40 scores and〈20 scores) according to the Gensini scores of coronary arteries lesion. The analysis of comparison and correlation was performed to estimate the relationship between BNP and cTnT level of the different Gensini scores of coronary arteries lesion. Results The BNP and cTnT level of the Gensini scores of coronary arteries lesion in ACS shown that Gensini scores of 〉40scores group was higher than that of 20-40 scores group, 20-40scores group higher than that of 〈 20scores group, which was significantly different (P〈0. 000- 0. 021 ). The analysis of Sperman correlation showed Gensini scores were positively correlated with the BNP level and cT- nT level(r=0. 288 and r=0. 354, P〈0. 001). Conclusion The serum BNP and cTnT level changes of ACS may reflect the difference of the extent and severity of myocardial necrosis and heart function synchronous reaction. It has a certain reference value for clinical diagnosis and treatment strategy and prognostic evaluation.
