摘要
目的观察光动力治疗尖锐湿疣的临床疗效和安全性,以进一步提高本病的临床疗效。方法选取在我院皮肤科就诊的尖锐湿疣患者180例,采用随机数字表法将其分为A组、B组和C组各60例。A组采用光动力疗法进行治疗,B组采用二氧化碳激光进行治疗,C组采用液氮冷冻进行治疗。采用荧光聚合酶链反应检测人乳头状瘤病毒(HPV-DNA)的清除情况。对比三组患者治疗后的疣体复发率和不良反应发生的情况。结果治疗6个月后,A组的病毒清除率为85.00%,B组的病毒清除率为55.00%,C组的病毒清除率为51.67%。B组和C组的病毒清除率均显著低于A组(P<0.05)。治疗后,A组的总复发率为11.67%,B组的总复发率为33.33%,C组的总复发率为38.33%。B组和C组的复发率均显著高于A组(P<0.05)。在治疗过程中,B组的不良反应发生率为41.67%,C组的不良反应发生率为50.00%,均显著高于A组(11.67%,P<0.05)。结论光动力疗法治疗尖锐湿疣具有良好的临床疗效,且复发率低,不良反应少,是尖锐湿疣较为理想的治疗方法,值得进一步推广应用。
Objective To observe the clinical efficacy and safety of photodynamic therapy in the treatment of condyloma acuminatum, for further enhancing the clinical curative effect of the disease. Methods 180 patients with condyloma acuminatum were selected in our hospital. They were randomly divided into group A, group B and group C , 60 cases in each group. Group A was treated with photodynamic therapy, group B with CO2 laser treatment, group C treated by liquid nitrogen freezing. The eradication of human papilloma virus ( HPV - DNA) was determined by using the method of fluorescent polymerase chain reaction. The differences of recurrence rate and adverse effects were compared among the three groups. Results After treated for 6 months , virus removal rate of group A was 85%, and significantly higher than that of group B (55%) and groupC (51. 67%) ,(P〈0. 05). The total relapse rate in group A, B and C were 11.67%, 33.33% and 38. 33% respectively, total relapse rate of group B and C were significantly higher than in group A ( P 〈 0. 05 ). During the treatment, adverse reactions occurrence rate of group B and Cwere 41.67% and 50% , significantly higher than that of group A ( 11. 67% , P 〈0. 05 ). Conclusions Photodynamic therapy was effectiveon condyloma acuminatum for the low recurrence rate and less adverse reaction, worthy of further application.
