摘要
目的比较丙泊酚与七氟烷对心脏瓣膜置换术患者诱导期及气管插管时的影响。方法将84例心脏病拟行心脏瓣膜置换术的患者根据麻醉方式不同分为丙泊酚组和七氟烷组,每组42例,丙泊酚组采用丙泊酚麻醉,七氟烷组采用七氟烷麻醉,比较2组患者的血流动力学指标变化情况。结果经重复测量设计的方差分析,丙泊酚组患者各指标与七氟烷组比较差异有统计学意义(P<0.05);经两两比较其结果显示,丙泊酚组患者T2、T3、T4、T5时的心率(HR)、中心静脉压(CVP)均高于七氟烷组(P<0.05),T3、T4、T5时的平均动脉血压(MAP)高于七氟烷组(P<0.05),T3、T4时的心脏指数(CI)高于七氟烷组(P<0.05),差异均有统计学意义;2组患者T2、T3、T4、T5时的HR、MAP均低于治疗前,丙泊酚组患者T3、T4、T5时的CVP,T4时的CI高于治疗前,七氟烷组患者T3、T4、T5时的CVP,T1时的CI也高于治疗前,差异均有统计学意义(P<0.05)。结论丙泊酚和七氟烷对心脏病患者全身麻醉气管引起的心血管反应均有抑制作用,丙泊酚使诱导期血流动力学更平稳。
Objective To compare the effect of propofol and sevoflurane on induction period and tracheal cannula period in patients with heart valve replacement. Methods Eighty-four patients with heart disease who underwent heart valve replacement were divided into propofol group and sevoflurance group according to the anesthesia way,with forty-two patients in each group. The propofol group received propofol anesthesia,the sevoflurance group received sevoflurane anesthesia; the changes of hemodynamic indexes were compared between the two groups. Results There was statistic difference of each index between the propofol group and sevoflurance group( P〈0.05); the heart rate( HR) and central venous pressure( CVP) at T2,T3,T4 and T5 moment in propofol group were greater than those in sevoflurance group( P〈0.05); the mean arterial blood pressure( MAP) at T3,T4,T5 moment in propofol group were greater than those in sevoflurance group( P〈0.05); the cardiac index( CI) at T3 and T4 moment in propofol group were greater than those in sevoflurance group( P〈0.05). Compared with before propofol,the HR and MAP at T2,T3,T4 and T5 moment in the two groups were decreased( P〈0.05); the CVP at T3,T4 and T5 moment and CI at T4 moment in propofol group were increased( P〈0.05); the CVP at T3,T4 and T5 moment and CI at T1 moment in sevoflurance group were increased( P〈0.05). Conclusion Propofol and sevoflurane has the inhibitory effect on cardiovascular reaction caused by general anaesthesia in patients with heart disease. The propofol makes the hemodynamic more smooth in induction period.
