摘要
目的 探讨关节镜下清理结合黏弹性补充治疗膝关节退行性骨关节病的临床效果。方法 选取2011年10月～2012年12月北京中医药大学东直门医院东区骨伤科就诊的膝关节退行性骨关节病患者50例，所有患者随机分为研究组和对照组，每组各25例。观察及比较两组膝关节功能评分、疼痛评分、力量评分、疗效、视角模拟尺度评分（VAS）及全身症候积分。结果 研究组患者膝关节功能评分、疼痛忍受评分[（82．19±11．37）、（19．49±4．16）分]高于对照组[（71．39±10．19）、（14．37±3．16）分]，差异均有统计学意义（P〈0．05）；而力量评分，研究组为（6．51±1．17）分，对照组为（6．44±1.31）分，两组比较，差异无统计学意义（P〉0．05）。研究组患者总有效率为92．0％，对照组为76．0％，两组比较，差异有统计学意义（P〈0．05）。研究组治疗前后VAS、全身症候积分评分比较，差异有统计学意义（P〈0．05）；治疗后研究组VAS、全身症候积分评分分别为（1．53±0．11）、（8．61±0．37）分，对照组分别为（2．14±0．21）、（13．29±1．12）分，两组比较，差异有统计学意义（P〈0．05）。结论 关节镜下清理结合黏弹性补充治疗膝关节退行性骨关节病效果确切。值得临床推广应用。
Objective To explore the clinical curative effect of arthroscopic debridement combined with viscoelasticity supplement in the treatment of degenerative knee osteoarthritis. Methods 50 patients with degenerative knee osteoarthritis from October 2011 to December 2012 in East of Dongzhimen Hospital of Beijing University of Chinese Medicine were collected, all patients were randomly divided into the study group and the control group with 25 cases in each group. The knee joint function score, pain score, power score, curative effect, VAS and systemic symptoms score between the two groups were observed and compared. Results The knee joint function score and pain score of patients in the study group [（82.19±11.37）, （19.49±4.16） points] respectively were all higher than those in the control group [（71.39± 10.19）, （14.37±3.16） points respectively], the differences were statistically significant （P 〈 0.05）; while the power score in the study group were （6.51±1.17） points, that in the control group were （6.44±1.31） points, the difference was not statistically significant between the two groups （P 〉 0.05）. The total effective rate in the study group and control group was 92.0%, 76.0% respectively, the difference was statistically significant between the two groups （P 〈 0.05）. The difference was statistically significant in VAS, systemic symptoms score of the study group between before and after treatment （P 〈 0.05）; The VAS and systemic symptoms score in the study group after treatment were （1.53±0.11）, （8.61± 0.37） points respectively, those in the control group were （2.14±0.21）, （13.29±1.12） points respectively, the differences were statistically significant between the two groups （P 〈 0.05）. Conclusion Arthroscopic debridement combined with viscoelasticity supplement in the treatment of degenerative knee osteoarthritis has exact clinical curative effect, and it is worthy of clinical application.
