摘要
目的探讨耐甲氧西林金黄色葡萄球菌(MRSA)和甲氧西林敏感金黄色葡萄球菌(MSSA)感染造成的经济损失。方法回顾性调查2012年1月一2013年12月456例MRSA感染和250例MSSA感染患者的住院费用与住院天数,并按照1:1匹配设立对照组,比较各组的住院费用和住院天数。结果MRSA感染组、MSSA感染组住院费用分别增加82694.8元和18735.3元,延长住院天数14.1d和5.9d;ICU中MRSA感染增加费用最高,为153344.5元,延长住院天数则以外科明显;60~75岁MRSA造成的直接经济损失最高,达100208.9元,延长住院天数22d;MSSA感染造成的经济损失则以〉75岁组最大,直接经济损失23914.4元,延长住院天数6.9d。结论MRSA感染增加患者经济负担,延长住院时间,同时也影响医院病床周转率。
OBJECTIVE To evaluate the direct economic losses caused by the methicillin-resistant Staphylococcus aureus (MRSA) infection and the methicillin-sensitive Staphylococcus aureus (MSSA) infection. METHODS A total of 456 patients with MRSA infection and 250 patients with MSSA infection, who were treated in the hospital from Jan 2012 to Dec 2013, were enrolled in the study, then the hospitalization cost and the length of hospital stay of the subjects were retrospectively investigated, the control group was set up in a 1 : 1 ratio, and the hospitalization cost and the length of hospital stay were compared. RESULTS The hospitalization cost of the patients with MRSA infections increased by 82 694.8 yuan, the patients with MSSA infections 18 735.3 yuan; the prolonged length of hospital stay of the patients with MRSA infections was 14.1 days, the patients with MSSA infection 5.9 days; the increase of the hospitalization cost of the patients with MRSA infections was highest in the ICU ( 153 344.5 yuan), and the prolonged length of hospital stay was longest in the department of surgery. The direct economic losses caused by the MRSA infection in the patients aged between 60 and 75 years old was the highest ( 100 208.9 yuan), with the prolonged length of hospital stay of 22 days. The economic losses caused by the MSSA infection in the patients aged more than 75 years old was the highest, with the direct economic losses of 23 914.4 yuan, the prolonged length of hospital stay of 6.9 days. CONCLUSIONS The MRSA infection increases the economic burden of the patients, extends the length of hospital stay, and influence the turnover rate of the bed.
