成人楔状缺损修复疗效及影响因素分析 被引量:5

Influencing factors analysis of restoration of teeth with wedge-shaped defects

摘要 目的探讨几种不同材料修复成人楔状缺损的疗效并分析其影响因素。方法将60例楔状缺损患者的320颗患牙随机分为4组,分别使用玻璃离子、光固化复合树脂、光固化玻璃离子水门汀和复合体Dyract进行修复,2年后观察成功率,并分析患者年龄、性别、缺损情况、牙位、是否偏侧咀嚼、咬合是否正常等因素是否对成功率有影响。结果传统玻璃离子组成功率是88.8%(71/80)、光固化复合树脂组成功率是89.3%(75/84)、光固化玻璃离子水门汀组成功率是92.3%(72/78)、复合体Dyract组成功率是97.4%(76/78),复合体Dyract组的成功率最高,与传统玻璃离子组、光固化复合树脂组比较差异有统计学意义(χ2=4.600、4.245,P<0.05),其他各组组间比较差异均无统计学意义(P>0.05);年龄小于50岁、缺损较轻、非上前磨牙、双侧咀嚼及咬合正常可以显著增加楔状缺损修复的成功率(P<0.05),而性别则不影响成功率(P>0.05)。结论临床选择充填材料时,确定了患者接受度之后,对于年龄≥50岁、缺损情况较为严重、上前磨牙、偏侧咀嚼及咬合不正常的患者,考虑尽量使用Dyract复合体和光固化玻璃离子水门汀以提高换牙修复的成功率。 Objective To evaluate the utility of five restorative materials used for adult wedge-shaped defect in teeth and its impact factors. Methods Total 320 teeth with wedge-shaped defects in 60 patients were randomly divided into 4 groups,and were repaired with glass ionomer,light-cured composite resin,light-cured glass ionomer cement and Dyract,respectively. Two years follow up was carried out to observe the success rate. The correlations of age,gender,defect,tooth bit,tooth position,hemimastication and occlusion with the success rate were analyzed. Results The success rate in the glass ionomer group was 88. 8%( 71 /80),in light-cured composite resin group was 89. 3%( 75 /84),in light-cured glass ionomer cement group was 92. 3%( 72 /78),and in Dyract group was 97. 4%( 76 /78). The difference between the Dyract group and conventional glass ionomer group or light-cured composite resin group was statistically significant( χ^2= 4. 600, 4. 245,P〈0. 05); there were no difference among the others( P〈0. 05). When the patients was younger than 50 years old,or with lighter defects,or the tooth was not upper premolars,with bilateral chewing and normal occlusion,the success ratio of repair should be improved for the teeth with wedge-shaped defects( P〈0. 05). The gender did not affect the success rate( P〈0. 05). Conclusion According to the acceptance of patients,light-cured glass ionomer cement and Dyract should be the preferred choice for the patients aged more than 50 years,with serious defect,upper premolars,hemimastication and abnormal occlusion,thus should improve the success rate of repair.
