右美托咪定联合丙泊酚、芬太尼用于无肌松药下全麻气管插管 被引量:7

摘要 目的评价右美托咪定联合丙泊酚、芬太尼在无肌松药情况下全麻气管插管的条件。方法择期腹腔镜下子宫切除术患者60例随机分为两组(n=30),A组:静脉注射丙泊酚2mg/kg、芬太尼0.2μg/kg、维库溴铵1mg/kg麻醉诱导气管插管;B组:静脉注射右美托咪定1μg/kg、丙泊酚2mg/kg、芬太尼0.2μg/kg麻醉诱导气管插管。分别于诱导前、插管前即刻、插管后即刻、插管后1min、5min记录心率、血压和脑电双频谱指数(BIS),计算心率和血压变异系数。根据面罩通气难易程度、下颌松弛程度、声带位置、患者对插管和气管导管套囊充气时的反应来评价气管插管条件。结果 A组心率变异系数小于B组(P<0.05),B组血压变异系数小于A组(P<0.05)。两组各时间点BIS均低于给药前(P<0.05),组间没有明显区别(P>0.05)。A组声带位置、患者对插管和气管导管套囊充气时的反应优于B组(P<0.05),面罩通气、下颌松弛两组没有明显区别(P>0.05)。B组插管总的优良率低于A组(P<0.05)。结论无肌松药下右美托咪定联合丙泊酚、芬太尼气管插管基本能满足插管条件。
