摘要
目的探讨老年高血压患者认知功能与胰岛素抵抗(IR)的关系。方法对132例老年高血压患者分为两组:合并认知功能损害患者组61例,认知功能正常患者组71例,比较两组间性别、年龄、文化程度、BMI、WHR、TC、TG、CRP、HDL-C、LDL-C、Cr、HOMA-IR、SBP和DBP的差异,并进行Logistic回归分析。结果老年高血压合并认知功能损害组较老年高血压认知功能正常组文化程度较低(文化程度大学及以上:44.26%vs.67.61%,P<0.01),BMI[(26.8±2.2)kg/m2vs.(22.9±2.3)kg/m2,P<0.01]、WHR(0.93±0.06vs.0.81±0.05,P<0.01)、TG[(2.3±0.6)mmol/Lvs.(1.7±0.5)mmol/L,P<0.01]、FPG[(6.1±1.3)mmol/Lvs.(5.2±0.9)mmol/L,P<0.01]、FINS[(21.7±12.9)mU/Lvs.(9.3±3.7)mU/L,P<0.01]、HO-MA-IR(5.9±3.1vs.2.1±1.3,P<0.01)较高,差异有统计学意义,Logistic回归分析显示文化程度(P<0.01)、BMI(P<0.05)、WHR(P<0.05)、HOMA-IR(P<0.01)是认知功能损害的独立影响因素。结论文化程度、BMI、WHR、HOMA-IR是老年高血压认知功能损害的独立影响因素。高的文化程度是一个保护性因素,而其他几项都是危险因素。IR是老年高血压患者认知功能障碍重要的危险因素。
Objective To explore the relationship between cognitive impairment and insulin resistance in elderly patients with hypertension.Methods 132 patients were divided into two groups：cognitive impairment group （n =61） and non-cognitive impairment group （n =71）,gender,age,cultural level,BMI,WHR,TC,TG,CRP,HDL-C,LDL-C,Cr,FPG,FINS,HOMA-IR,SBP and DBP were compared between the two groups,and logistic regression was analyzed.Results In cognitive impairment group,cultural level was lower（44.26% vs.67.61%,P 〈 0.01）,thelevel of BMI[（26.8 ±2.2） kg/m2 vs.（22.9 ± 2.3）kg/m2,p 〈0.01],WHR（0.93 ±0.06 vs.0.81 ±0.05,P 〈0.01）,TG[（2.3 ±0.6）mmol/L vs.（1.7 ±0.5）mmol/L,P 〈0.01],FPG[（6.1 ± 1.3） mmol/L vs.（5.2 ±0.9）mmol/L,P 〈0.01],FINS [（21.7 ± 12.9）mU/L vs.（9.3 ±3.7）mU/L,P 〈0.01],HOMA-IR（5.9 ±3.1 vs.2.1± 1.3,P 〈 0.01）were higher than the control group.In the Logistic regression,cultural level （P 〈 0.01）,BMI （P 〈0.05）,WHR（P〈0.05）and HOMA-IR（P 〈0.01） were independent factors.Conclusions The cultural level,BMI,WHR and HOMA-IR are independent factors of cognitive impairment in elderly patients with hypertension.Education is a pretective factor and the others are risk factors.Insulin resistance is an important risk factor for the cognitive impairment in primary elderly hypertensive patients.
