摘要
目的 应用99Tcm 亚甲基二膦酸盐 (MDP)显像技术评估高压电损伤程度。方法 3组兔分别为高压电损伤轻、重和毁损标准动物模型 ,伤肢分别进行99Tcm MDP三相显像、解剖探察和组织学观察。结果 轻度击伤肢血池相及血流相均显示局部放射性浓聚 ,组织学检查证实为细胞轻度水肿 ,微血管栓塞 ;重度损伤肢体远端影像淡而不清 ,并在近端影像骤然增强 ,组织学检查为肌间隔性溶解碎裂 ,多数血管栓塞 ;毁损伤肢动脉血流灌注中断 ,影像的界面平整如刀割样 ,镜下肌大片溶解坏死。结论 99Tcm MDP与组织损伤程度影像一致 ,与组织学检查和临床表现相符 ,可较准确反映不同程度的损伤。
Objective To study the application of the 99 Tc m MDP imaging in diagnosis of high voltage electrical trauma. Methods Three groups of model rabbits with mild,severe and destructive electric trauma,respectively were used in the study,and the extents of the traumas were evaluated using three phase 99 Tc m MDP imaging,vivisectional and histological. Results Blood flow imaging and pool imaging in the slightly injured limb showed thicker or black tracer accumulation,and in histological study it showed slight cell swell,capillary embolism.In serious injured limb the images were thin and blurred with obvious thicker images in proximal side,histological results revealed the uncontinuous necrosis and necrolysis of the muscle and multivessel embolism.The arteries of the destroyed limb showed blocked arteries with clear edges in images,histological study showed whole muscle necrosis and multivessel embolism. Conclusion The tracer uptake in three phase 99 Tc m MDP imaging is decreasing concordantly with the intensity of the electrical injury and this technique is very useful for the diagnosis of high voltage electrical trauma.
