摘要
目的对人工肱骨头置换术与切开复位钢板内固定治疗肱骨近端粉碎性骨折的疗效进行对比研究。方法选取2010年1月至2012年12月在本院进行手术治疗的肱骨近端骨折患者49例,其中,男性21例,女性28例,分为人工肱骨头置换术组(27例)和切开复位钢板内固定组(22例),对两组患者的手术时间、手术出血量、Nerr评分以及Constant-Murley评分结果等疗效指标进行观察对比。结果两组患者在手术时间和手术出血量方面存在显著差异(P<0.05),即人工肱骨头置换组较切开复位钢板内固定组的手术时间长,手术出血量大。而两组患者在Nerr评分方面无显著差异(P>0.05);两组患者的肩关节疼痛、日常生活能力、肩关节活动度和上肢肌力之间无显著差异(P>0.05)。结论人工肱骨头置换术治疗肱骨近端粉碎性骨折与切开复位钢板内固定治疗效果接近,前者具有手术时间短、出血量少的优点,值得临床进一步推广使用。
ObjectiveTo explore the clinical curative effect between artificial humerus head replacement and internal ifxation for treatment of comminuted proximal humeral fractures.MethodsChoose 49 patients with surgical treatment of proximal humeral fractures from January 2010 to December 2010 in our hospital, there were 21 men and 28 women, artiifcial humerus head replacement group had 27 cases, the internal ifxation group had 22 cases, then recorded the two groups curative effect of operation time, surgical blood loss, Nerr score and Constant-Murley score results.ResultsThere were significant differences in operation time and blood loss in operation of two groups （P〈0.05）, the artiifcial humerus head replacement group had more operation time and less blood loss in operation than the internal ifxation group. And two groups of patients in Nerr score had no signiifcant differences （P〉0.05）; there was no signiifcant difference between two groups of shoulder joint pain, daily life ability, shoulder joint mobility and upper extremity strength （P〉0.05）.ConclusionThe treatment effect of artiifcial humerus head replacement group is close to the internal ifxation group, the former has the advantages of shorter operation time and less blood loss, it is worth further clinical use.
