慢性充血性心力衰竭患者淋巴细胞β肾上腺素能受体密度与血小板钙含量相关性探讨

Relationship between changes of lymphocyte β adrenoceptor densities and platelet cytosolic calcium concentration in patients with chronic congestive heart failure

摘要 目的 研究慢性充血性心力衰竭 (CHF)患者淋巴细胞 β肾上腺素能受体 ( β AR)密度是否与细胞内高钙负荷有关。方法  2 6例CHF患者及性别、年龄结构相似的正常健康人 15名 ,取晨间清醒空腹状态下静脉血 ,用3 H 心得舒放射配基受体结合法测定淋巴细胞 β AR密度 ;以Fura2 AM作为荧光指示剂 ,用双波长荧光比值法测定血小板细胞内游离钙浓度 (PCC) ;用12 5I放射免疫法测定血浆去甲肾上腺素 (NE)水平。结果 心衰组 β AR明显低于对照组 (P <0 0 0 1) ,PCC与血浆NE水平均显著高于对照组 (P <0 0 0 1)。相关分析结果表明 ,心衰组 β AR与PCC呈显著负相关 (r2=0 5 8,P <0 0 0 1) ,与血浆NE水平无显著相关性 ;对照组 β AR无论与PCC还是血浆NE水平均无显著相关性。结论 CHF患者 β AR密度下降与细胞内钙含量增高有关。 Objective To determine whether overload cytosolic calcium ion was involved in down regulation of β adrenoceptor densities in patients with chronic congestive heart failure(CHF).Methods 26 Patients with CHF (average age 56±7 5ys)(NYHA Ⅱ,Ⅲ)were randomly admitted to the study.15 Additional subjects (average age 55±5 6ys),with no history of cardiac disease,hypertension or other cardiovascular disorders,had early morning resting blood specimens obtained as control.[ 3H] Dihydroalprenolol (DHA)radioligand binding assay was used to measure lymphocyte β AR density,fluorescent indicator Fura2 AM to estimate platelet cytosolic calcium concentration (PCC) and 125 I radioimmunoassay to measure plasma norepinephrine (NE)levels.Results β AR densities in CHF patients were lower than that in control group ( P< 0 001).PCC and plasma NE levels in CHF patients were significantly higher than that in control group ( P< 0 001,respectively).Results of correlation analysis showed that there was significant negative correlation between β AR and PCC in CHF patients (r 2=0 58, P< 0 001).But there was no significant correlation between β AR and plasma NE levels in CHF patients.In control group,no significant correlation either between β AR and PCC or between β AR and plasma NE levels was observed in this study.Conclusion Overload cytosolic calcium ion was involved in the down regulation of β adrenoceptor densities in CHF patients.
