摘要
目的 提高TUR P手术前列腺增生组织的切除率。 方法 回顾性分析 34 6例TUR P手术切除前列腺增生组织重量及比例。 结果 34 6例手术平均切除前列腺增生组织重量为2 8.7g ,平均切除率为 46 .2 %。切除率与估计重量呈正相关 (P <0 .0 0 1)。熟练操作者和普通操作者的切除率差别无显著性意义 (P >0 .0 5 )。 结论 TUR P手术切除比例随前列腺体积增大而增加 ,残留的增生腺体是限制手术切除比例的原因。
Objective To determine the proportion of prostate that we have resected during transurethral resection of the prostate (TUR-P). Methods The patients undergone TUR-P from January 1997 to December 1999 were studied. The prostate estimated weight was calculated by the length of anteroposterior and transverse and craniocaudal diameters measured by transabdominal ultrasonography before operation. The prostate tissue collected at resection was weighed, multiplied by 1.2 to compensate for “shrinkage,” and the proportion of the prostate resected was the percentage of that of the pre-operative estimated weight. Results For the 346 patients studied with the age ranged between 52 and 85, the mean pre-operative estimated weight of the prostate was 59.6 g. Over 60 percent,the estimated weight was from 40 g to 70 g.The mean weight of resected tissues was 28.7 g while the mean percentage of resection was 46.2%. There is a positive correlation between the percentage of resection and the prostate volume (P<0.001). There are no differences of the ratios of resection between the skilled operators who have been much more experienced in TUR-P and the common operators (P>0.05). Conclusions It seems no differences between this study and the studies reported before in the proportion of resection during TUR-P. This study demonstrates that in most cases the resection of prostate is no more than half of the gland and it is impossible to remove the gland completely. While greater resection ratios were obtained in larger glands, this implied that in these glands there was more hyperplysia tissue, which occupied more proportion of the total prostate weight. Even the skilled operators could not remove more proportion of the glands.
