摘要
目的 评估彩色室壁动力分析技术 (CK)低剂量多巴酚丁胺负荷超声 (LDDSE)识别存活心肌的价值。方法 2 0例经冠状动脉造影术 (CAG)证实为冠心病 (CHO)的患者 ,术前全部行CK LDDSE检查 ,并与经皮冠状动脉腔内成形术 (PTCA)后实际改善的心肌节段进行对比研究。结果 在PTCA术前行CK LDDSE检查的 32 0个节段中 ,96段无运动 ,术后CK检查有收缩功能储备的 78个节段中 ,术前CK能检查出 70个节段 ,CK LDDSE预测存活心肌的敏感性 89 7% ,特异性 77 8% ,准确性 87 5 %。结论 CK LDDSE识别存活心肌客观、准确、安全 ,具有较高的临床应用价值。
Objective To assess the value of color kinesis low dose dobutamine stress echocardiography(CK LDDSE) in identifying viable myocardium.Methods 20 patients with coronary heart disease(CHD) proved by coronary artery angiography(CAG) were studied.CK LDDSE was performed before percutaneous transluminal coronary angioplasty(PTCA).The identified viable myocardium by CK LDDSE was compared with the actual improvement detected after PTCA.Results Before PTCA,of all the 320 segments,96 were akinetic,and of the 78 segments which had contractile reserve function,70 were detected.After PTCA,CK LDDSE was able to predict improvement in 70 of the 78 segments that recovered their function after revascularization(sensitivity=89 7%) and to identify 14 of the 18 segments that did not recover postoperatively(specificity=77.8%);thus the accuracy was 87 5%.Conclusion CK LDDSE provides an objective,accurate and safe method for identifying viable myocardium.This technique might be of considerable value clinically.
