激光心肌血运重建术结合转VEGF基因对缺血心肌侧枝循环的影响 被引量:1

Therapeutic effect of transmyocardial laser revascularization combined with VEGF gene on collateral circulation for myocardial ischemia

摘要 为明确激光心肌血运重建术 (Transmyocardiallaserrevascularization ,TMLR)与转血管内皮生长因子 (Vascularen dothelialgrowthfactor,VEGF)基因相结合能否促进侧枝循环建立 ,我们在猪的急性心肌缺血模型上 ,用冠脉造影观察了TMLR、TMLR +VEGF对缺血心肌侧枝循环的影响。结果显示 ,与术后即刻或单纯缺血组相比 ,TMLR(Rentrop记分 1 4± 0 9)、TMLR +VEGF(Rentrop记分 2± 0 5 )均能显著促进侧枝循环建立 (P均 <0 0 5 ) ;TMLR +VEGF组较TMLR组侧枝有增多的趋势。分时间点统计显示 ,TMLR +VEGF组 4周为 2± 0 8,8周为 2± 0 0 ,TMLR组 4周为 2± 0 0 ,8周为 1± 1 0。其中TMLR +VEGF组 4周、8周和TMLR组 4周结果无差异 ,而TMLR组 8周与其他各组相比均呈下降的趋势。提示与TMLR相比 。 To investigate whether TMLR combined with VEGF gene therapy may further improve collateral circulation induced by TMLR alone, fifteen pigs were randomized to TMLR+VEGF group (n=7) or sole TMLR group (n=5) or ischemic group (n=3) Acute myocardial ischemia was created in porcine hearts by ligating left anterior descending artery In both TMLR+VEGF and TMLR group, channels were performed on ischemic myocardium with Q switched YAG laser Intramyocardial injections were placed in the vicinity of channels The TMLR+VEGF group used pcD 2/VEGF 121 expression plasmid(800μL, 2 5mg/mL) The sole TMLR group used sterile distilled water (800μL) Animals were sacrificed 4 weeks or 8 weeks post therapy According to Rentrop score of coronary angiogram, TMLR+VEGF group(2±0 2) and sole TMLR group(1 4±0 4) were significantly higher compared with ischemic group (0 5±0 5)(both P <0 05) TMLR+VEGF group was 2±0 4 four weeks post therapy and 2±0 0 eight weeks post therapy; sole TMLR group was 2±0 0 four weeks post therapy and 1±0 6 eight weeks post therapy The Rentrop score for sole TMLR group eight weeks post therapy has a trend to decrease (0 05< P <0 10) Compared with TMLR alone, TMLR combined with VEGF gene therapy can stabilize collateral circulation to some extent
