摘要
目的探讨下肢静脉曲张伴原发性下肢深静脉瓣膜功能不全的治疗方式。方法回顾性总结 1990年 11月至 2 0 0 0年 6月间治疗的下肢静脉曲张伴有深静脉瓣膜功能不全 (3～ 4)级患者189人 ,2 45条肢体。将所有患者随机分为试验组 133条肢体和对照组 112条肢体 ,所有患者均行大隐静脉高位结扎剥脱术或大隐静脉高位结扎电凝术 ,试验组同时行股浅静脉戴戒术。结果手术成功率 10 0 % ;随访率为 87% ,随访时间为 6个月至 9年 2个月 ,平均 4年 ;两组术后的复发率均为 3 3% ,症状及体征有极大的改善 ,疗效比较差异无显著意义 (P >0 0 5 )。结论下肢静脉曲张伴有原发性下肢深静脉瓣膜功能不全时 ,单独进行浅静脉的高位结扎剥脱术即能达到满意的结果 。
ObjectiveTo evaluate the surgical treatment for varicose veins with deep venous insufficiency of the lower extremity. Methods From Nov.1990 to Jun 2000,189 patients (245 limbs) with varicose veins of the lower extremity accompanying deep vein reflux of 3～4 grade were treated in our department. Patients were randomly divided into trial group(133 limbs) and control group(112 limbs) receiving high ligation with stripping of the great saphenous vein in the control group, and additional external banding valvuloplasty of the superficial femoral vein in the trial group. ResultsThe operation was 100% successful in the two groups, 87% of the postoperative patients was followed up with an average of 4 years (ranging from 6 months to 9 years and 2 months). The recurrence rate in the two groups was the same (3.3%), and symptoms and signs had a great improvement. The two groups were not significantly different in treatment efficacy (P>0.05). ConclusionsWhen varicose veins of the lower extremity with deep vein insufficiency exists, patients should undergo high ligation and stripping of the great saphenous vein, additional correction of the deep vein reflux provides no further effect.
