上颌第一磨牙根管口形态、位置的应用解剖 被引量:17

摘要 目的 研究上颌第一磨牙根管口形态和位置关系 ,为根管治疗时寻找根管口提供解剖学依据。方法 采用目镜配置测微尺的XTL 2型体视摄影显微镜 ,观测 42个上颌第一磨牙的根管口形态及其与髓室底截面形态和髓室底沟的位置关系。结果 ①上颌第一磨牙根管口可呈长条形、卵圆形、圆形、哑铃形和肾形 ;80 .95 %含3个根管口 ,其远颊根管口孔径最小。②近颊根舌侧根管口的出现率为 14 .2 9%。③髓室底截面呈长方形和圆三角形的占 76.19% ,根管口多靠近相应髓室壁构成的夹角附近。④髓室底沟分五型 ,80 .96%的沟连接 3个根管口 ,呈“Y”形或“7”形。结论 ①寻找根管口可综合髓室壁夹角、根管口间距及髓室底沟的走向进行 ;②上颌第一磨牙的第四根管口的出现率约为 15 %。 Objective To study the shape and location of root canal orifice in maxillary first permanent molar and provide a morphological basis for locating canal orifice in endodontic therapy. Methods The shape of root canal orifice and its relation to pulp chamber walls and the groove on pulpal floor in 42 maxillary first permanent molor were observed and measured under the XTL 2 photostereograph microscope. Results ①The shape of opening for root canal was classified into five types:stripform,oval,round,dumbbell and renal. 80.93 % of teeth had three root canal orifices ,of which the diameter of distobuccal canal orifices was the smallest. ②The mesiobuccal root had two canal orifices 14.29 % of the time. ③The shape of the pulpal floors in transverse cutting plane was classified into four types:rectangle,round triangle,dumbbell and oval.④ The groove on the pulpal floors was classified into five types,of which the 'Y' and '7' shaped accounted for 80.96 %. Conclusion ①The angle formed by the pulp chamber walls,the distance between the canal orifices and the direction of grooves on the pulp chamber floors can provide a manual in locating the openings for the root canals. ②The incidence of mesiolingual canal orifice is approximatelly 15% of the time.
