摘要
目的 :探讨晚期妊娠孕妇服用中药 :当归 30g、川芎 15g、龟板 15g、艾叶 15g组成的汤药促宫颈成熟的临床效果及可行性。方法 :选择晚期妊娠、宫颈Bishop≤ 4分的初产孕妇共 148例 ,随机分成实验组 76例 ,对照组 72例。分别口服中药 ,每日 1剂 ,连用 3d和静脉推注蒂络安 2 0 0mg ,每日 1次 ,连用 3d。观察促宫颈成熟效果、诱引产兆效果、产程时间、分娩结果、副作用的情况。结果 :实验组与对照组在产程时间、新生儿情况及产后血量方面均无明显差异 (P >0 .0 5 ) ;两组促宫颈成熟效果 :实验组总有效率为 93.42 % ;对照组总有效率为 80 .5 6 % ,两组比较有显著性差异 (P <0 .0 5 )。两组诱引产兆效果 :实验组用药期间或停药 2 4h内自然临产 34例 ,有效率为 44 .74% ;对照组2 0例 ,有效率为 2 7.78% ,有显著性差异 (P <0 .0 5 )。结论 :中药当归、川芎、龟板、艾叶组成简单方剂 ,应用于晚期妊娠 ,既能促宫颈成熟 ,又能诱引产兆 ,两者同步化 ,明显优于蒂络安 ,是一种使用方便 ,安全有效的促宫颈成熟方法。
Objective: To explore the effectiveness and feasibility of Chinese herbal medicine consisted of Danggui 30g, Chuanxiong 15g, Aiye 15g and Guiban 15g on the cervical ripening in late pregnancy. Methods: One hundred and forty-eight primiparae with late pregnancy having bishop score ≤4 were divided randomly into experimental group (76 patients) and control group (72 patients), the herbs qd orally and sodium prasterone sulfate 200mg iv qd were given respectively for 3d. The effects on cervical ripening and inducing premonition, progress of labor, the safety of the mothers, fetuses and neonates and the adverse effects on the two groups were observed comparatively. Results: The progress of labor and the safety of mothers, fetuses and neonates showed no statistical difference between the two groups( P >0.05). But the effect on the cervical ripening had significant difference between two groups ( P <0.05), the total successful rates being 93.42% in the experimental group and 80.56% in the control group. About the effect on inducing premonition, the total effective rate was 44.74% in the experimental group and 27.78% in the control group( P <0.05). Conclusion: Chinese prescription containing Danggui, Chuanxiong, Aiye and Guiban used in late pregnancy is more advantageous in the cervical ripening and inducing premonition than sodium prasterone sulfate. It is a convenient, safe and effective way of cervical ripening.
