摘要
目的 探讨胃粘膜相关淋巴组织淋巴瘤的临床、内镜及病理特征 ,以提高早期诊断水平。方法 分析手术切除并经病理证实的 78例胃粘膜相关淋巴组织淋巴瘤患者的临床、内镜和病理资料。结果 患者男女比例为 1.0 5 :1,平均年龄 48.2岁 ,平均病程 2 5 .6个月 ,症状非特异性 ,以EⅠ2 和EⅡ1期病变为主。内镜检查 70例结果显示 ,病变多见于胃窦体 ,表现为溃疡性、隆起性及浸润性损害 ,以多灶性、多形性及弥漫性病变为特征。 5 6例胃活检中 ,组织学结合免疫组化可对 41例 (73 .2 % )术前确诊。 78例均为B细胞淋巴瘤 ,高恶性居多 ,67例 (85 .9% )幽门螺杆菌阳性。结论 胃粘膜相关淋巴组织淋巴瘤患者内镜改变具特征性。内镜下病变形态识别 ,多取。
Objective To investigate the features of clinical,endoscopic and pathologic of gastric mucosa associated lymphoid tissue lymphoma in order to improve diagnostic level in the early stage.Methods Clinical,endoscopic and histological data of 78 patients with gastric mucosa associated lymphoid tissue lymphoma diagnosed surgically and pathologically were reviewed and analyzed.Results The ratio of male to female of patients with gastric mucosa associated lymphoid tissue lympoma was 1.05 with mean aged 48.2 years and 25.6 months of clinical course.The symptoms were not specific.The tumours were divided into EⅠ 2 and EⅡ 1 stage predominantly and involved most commonly the antrum and body of stomach.70 cases showed ulcerative,bulgy and infiltrative lesions,and appeared multifocal,multimorphic and diffuse pathological changs under endoscope.Histologic examination of bioptic samples combined with immunohistochemistry attained 73.2% in 56 cases preoperationally.Immunohistochemistry showed B cell lymphoma in all the 78 cases among which were mainly high grade type.Helicobacter pylori was found in 67 cases (85.9%).Conclusion Gastric mucosa associated lymphoid tissue lymphoma are different from gastric cancar by its indolent clinical features and widespread,multifocal characters under endoscopy.It is essential to better understand endoscopic features of gastric mucosa associated lymphoid tissue lymphoma,try to get multiple,deeper bioptic samples combined with immunohistochemistry examination to improve diagnostic level in the early stage.
