摘要
[目的]观察柴胡疏肝散联合西药治疗急性结石性胆囊炎(肝气郁滞)疗效。[方法]使用随机平行对照方法,将104例门诊患者按就诊顺序号随机分为两组。对照组52例熊去氧胆酸,10mg/kg·次-1,2次/d,早晚餐后服用;阿莫西林,500g/次,3次/d;消旋山莨菪碱,10mg/次,3次/d。治疗组52例柴胡疏肝散(香附8g,甘草炙、枳壳各9g,川穹10g,柴胡12g,芍药18g),水煎400mL,1剂/d,早晚分服,大便秘结加大黄9g,番泻叶10g;小便短赤加车前15g;结石明显加海金沙15g,金钱草30g;胃脘满加青皮、陈皮、佛手各9g;胁痛甚加元胡9g,金铃子、郁金各12g;西药治疗同对照组。连续治疗2个月为1疗程。观测临床表现、疼痛程度评分、生活质量评分、不良反应。治疗1疗程(2个月),判定疗效。[结果]PRI、PPT、VAS两组均有降低(P<0.01),治疗组降低多于对照组(P<0.01)。社会、生理、心理、环境领域生活质量评分两组均有改善(P<0.05,P<0.01),治疗组改善优于对照组(P<0.01)。[结论]柴胡疏肝散联合西药治疗急性结石性胆囊炎(肝气郁滞),疗效满意,可明显改善疼痛程度,提高生活质量,无严重不良反应,值得推广。
[Objective]To observe the curative effect of Chaihu Shugan decoction combined with western medicine on acute calculous cholecystitis (Ganqi Yuzhi).[Methods] 104 outpatients were randomly divided into two groups according to the order of visiting.The control group (52 cases)was treated with ursodeoxycholic acid (10mg ·kg^-1,2 times a day)after breakfast and dinner,amoxicillin,500 g/time,3 times/day,racemic anisodamine,10 mg/time,3 times/day).In the treatment group,52 cases were treated with Chaihu Shugan decoction (Xiangfu 8g,Zhigancao,Zhiqiao each 9g,Chuanxiong 10g,Chaihu 12g,Chishao 18g),400mL decocted in water,1 dose per day,taken separately in the morning and evening,9g yellow stool constipation,10g Senna leaves,15g red stool,15g before plantain,15g Haijinsha,30g Chinensis,9g green peel,orange peel and bergamot respectively.Hypochondriac pain was 9g,Jinling and curcumin each 12g; western medicine treatment with the control group.Continuous treatment for 2 months for 1 courses.Clinical manifestations,pain scores,quality of life scores and adverse reactions were observed.1 courses of treatment (2 months)were used to determine the curative effect.[Results]PRI,PPT and VAS decreased in both groups (P<0.01),and the decrease in treatment group was more than that in control group (P<0.01).The scores of social,physiological,psychological and environmental quality of life in the two groups were improved (P <0.05,P<0.01),and the improvement in the treatment group was better than that in the control group (P <0.01).[Conclusion]Chaihu Shugan decoction combined with western medicine in the treatment of acute calculous cholecystitis (Ganqi Yuzhi)has satisfactory curative effect,can significantly improve the degree of pain,improve the quality of life,without serious adverse reactions,and is Worthy of promotion.
