摘要
目的探讨2型糖尿病合并急性脑梗死患者血清脂蛋白相关磷脂酶A2(Lp-PLA2)、白细胞介素(IL)-1β、IL-6及S100β与颈动脉斑块性质的相关性研究。方法回顾性分析我院收治的126例2型糖尿病合并急性脑梗死患者(A组),其中不稳定斑块64例,稳定斑块42例,无斑块20例。同期选择急性脑梗死患者60例(B组)及健康体检者60例(C组)。检测入院次日空腹Lp-PLA2、IL-1β、IL-6、S100β、血糖、糖化血红蛋白水平,用美国国立卫生研究院卒中量表(NIHSS)评估神经功能缺损,计算脑梗死体积,并进行分析。结果 A组Lp-PLA2、IL-1β、IL-6水平高于B组和C组[(337.86±94.67)μg/L vs(264.73±84.66)μg/L、(156.35±57.92)μg/L,(48.77±9.45)ng/L vs(35.02±7.03)ng/L、(10.02±5.77)ng/L,(30.12±5.45)ng/L vs(18.03±4.89)ng/L、(9.03±6.03)ng/L,P<0.05],B组Lp-PLA2、IL-1β、IL-6水平高于C组;A组NIHSS评分和脑梗死灶体积和S100β水平均高于B组(P<0.05)。A组不稳定斑块Lp-PLA2、IL-1β、IL-6、S100β、空腹血糖及糖化血红蛋白水平高于稳定斑块和无斑块患者(P<0.05);A组颈动脉斑块性质与空腹血糖、餐后2h血糖、糖化血红蛋白、Lp-PLA2、IL-1β、IL-6呈正相关。结论Lp-PLA2、IL-1β、IL-6和S100β可为2型糖尿病合并急性脑梗死患者病情评估、临床治疗及预后判断提供参考。
Objective To study the relationship of serum Lp-PLA2,IL-1β,IL-6and S100βlevels with carotid artery plaques in T2DM patients with acute cerebral infarction (ACI).Methods One hundred and twenty-six T2DM patients with ACI served as group A,60ACI patients served as group B and 60healthy people served as group C in this study.Their serum levels of Lp-PLA2, IL-1β,IL-6,S10013,FBG and HbAlc were measured,their neurological defect was scored and the size of their cerebral infarction was measured on day 2after admission.Results The serum levels of Lp-PLA2,IL-1βand IL-6were significantly higher in group A than in groups B and C,and in group B than in group C (P<0.05).The neurological score and serum S100βlevel were significantly higher and the size of cerebral infarction was significantly larger in group A than in group B (P<0.05).The serum levels of Lp-PLA2,IL-1β,IL-6,S100β,FBG and HbAlc were significantly higher in patients with unstable carotid artery plaques than in those with or without stable carotid carotid plaques (P<0.05).The nature of carotid artery plaques was positively related with the serum levels of FBG,2h postprandial blood glucose,HbAlc,Lp-PLA2,IL-1βand II-6in group A.Conclusion The serum levels of Lp-PLA2,IL-1β,IL-6and S100βplay an import role in formation,progression and stability of carotid artery plaques and can thus provide evidence for the assessment,treatment and outcome of T2DM patients with ACI.
