摘要
井夫杰教授认为椎动脉型颈椎病(vertebral artery type of cervical spondylosis,CSA)的形成分骨性因素和非骨性因素,骨性因素主要是骨赘形成,颈椎失稳,小关节紊乱等压迫或牵拉刺激椎动脉、交感神经,非骨性因素主要是软组织的炎症刺激,局部炎性物质的释放,刺激了椎动脉和交感神经,临证采用益脑推拿法治疗椎动脉型颈椎病,该法针推并重、标本兼治,注重头面、改善血流,筋骨并重、调节曲度,选穴精当、事半功倍,改变体位、操作安全,是治疗椎动脉型颈椎病行之有效的方法。益脑推拿法尤其适用于颈椎发生退行性病变化,动力平衡失调,颈椎椎节失稳,颈椎骨错缝引起的CSA。部分CSA患者如果存在某侧椎动脉整体纤细于对侧,或某侧椎动脉先天缺如等特殊改变的体征,则不是此手法治疗的最佳适应证。
Professor Jing Fu-jie believes that the osteogenesis and non-osseous factors lead to the formation of CSA. the osteogenesis factors are mainly due to osteophyte formation,cervical instability,facet joint disorders leading to compression or tracting and stimulating the vertebral artery and sympathetic nerve. The non-osseous factors are mainly due to inflammatory stimulation of soft tissue and release of local inflammatory substances stimulating the vertebral artery and sympathetic nerve. In clinical practice, massage therapy for nourishing brain is often used. This method pays attention to acupuncture and massage together,treating root and tips together,can improve head-face symptoms,blood circulation,tendons and bones,is an effective method to treat vertebral artery type cervical spondylosis by adjusting curvature,selecting acupoints precisely,doing twice the work with half the effort, changing posture and operating safely. This method is especially suitable for CSA caused by cervical vertebral instability, cervical spine fracture suture resulting from cervical dynamic imbalance with cervical degeneration. Some patients with CSA may not be the best indication for this manipulation if there are some special signs such as the whole vertebral artery of one side being thinner than that of the other side,or the congenital absence of the vertebral artery of one side.
