摘要
[目的]分析国内外成人住院患者营养风险筛查工具的特点及不同营养风险筛查工具的有效性,为临床工作者找到适合自己科室特色的筛查工具提供参考。[方法]系统检索美国肠外肠内营养学会网(ASPEN)、欧洲肠外肠内营养学会网(ESPEN)、CINAHL、PubMed、中国知网、万方和中国生物医学文献数据库(CBM)中与本研究密切相关的原始研究,只有报告了有效性或可靠性的文献才被纳入。由2名研究者独立筛选检索到的文献,并进行汇总分析。[结果]共纳入14篇文献,涉及14个筛查工具,工具内容主要基于年龄、BMI、最近有无体质量减少、食欲及疾病5个方面。每个工具信效度检测的标准不一,研究人群不尽相同,就所纳入的文献显示适用于老年群体的简版微型营养评价法(MNA-SF)的敏感度和特异度最高。[结论]所有的工具在与不同的参考标准进行比较时,信效度可能会不尽人意。临床工作者不能依赖某一个工具的筛查结果,临床观察也将在营养风险筛查中扮演重要角色。而将已有的工具适当调整以更适于所在专科特色是临床科研人员研究的方向。
[Objective]To analyze the characteristics and the effectiveness of nutritional risk screening tools for adult inpatients.[Methods]The original research of the nutritonal risk screening tools were systematically recruited from the American society for parenteral enteral nutrition network(ASPEN),the European society of parenteral enteral nutrition(ESPEN),CINAHL,PubMed,CNKI,Wanfang and CBM,and only the research with the validity and reliability were selected.Two researchers independently selected the retrieved literature and conducted the analysis.[Results]A total of 14 literatures were recruited,including 14 screening tools,most of which were based on age,BMI,recent weight loss,appetite and diseases.Every tool evaluated the reliability and validity with different standards as well as the study population was different.Our result showed that the mini nutritional assessment-short form(MNA-SF)had the highest sensitivity and specificity.[Conclusion]The reliability and validity of all tools may not be satisfactory when compared with different standards.Clinicians cannot rely on single tool,and clinical observation will always play an important role in nutritional risk screening.To adjust existing tools appropriately to suit their specialist departments may be the research direction in the future.
