锁定加压钩板与张力带治疗单纯外踝远端骨折的比较 被引量:4

摘要 [目的]比较锁定加压钩接骨板与克氏针张力带治疗简单外踝骨折的临床疗效。[方法]本院2010年3月~2017年3月收治单纯简单外踝远端骨折(横形、短斜形)患者42例,随机分为钢板组和张力带组,每组21例。其中,钢板组采用锁定加压钩接骨板治疗,张力带组采用克氏针张力带治疗。观察两组的治疗效果、术后踝关节功能和相关并发症情况。[结果]两组手术时长、术中出血量的差异无统计学意义(P>0.05),但钢板组术后下地负重时间明显早于张力带组,差异有统计学意义(P<0.05)。两组患者随访9~23个月,平均(15.27±7.93)个月。两组在骨折愈合时间、末次随访时踝关节ROM和临床结果优良率方面的差异无统计学意义(P>0.05)。钢板组术后总体并发症发生率为6.53%,张力带组为13.04%,两组间差异有统计学意义(P<0.05)。[结论]采用锁定加压钩接骨板和克氏针张力带治疗简单外踝远端骨折均能取得满意的临床疗效;但克氏针张力带组的术后并发症发生率高,且其术后下地负重康复时间晚。 [Objective] To compare the clinical outcomes of locking compression hook plate versus Kirschner wire tension band for simple distal lateral malleolar fracture. [Methods] Forty-two patients underwent surgical treatment in our hospital for simple distal lateral malleolus fractures from March 2010 to March 2017. The patients were randomly divided into the plate group and tension band group with 21 patients in each group, associated with corresponding internal fixation for the fractures.The clinical outcomes, ankle function and complications were compared between them. [Results] All patients in both groups had operation performed smoothly without serious intraoperative complications, such as neurovascular injuries. Although no statistically significant difference between the two groups in operation time and intraoperative blood loss(P>0.05), the plate group got significantly earlier ambulation postoperatively than the tension band group(P<0.05). The follow-up lasted for 9 to 23 months with an average of(15.27±7.93) months. There were no statistically significant differences between the two groups regarding to fracture healing time, as well as ankle range of motion, excellent and good rate of clinical outcomes based on Mazur’s grades at the latest follow up(P>0.05). However, the total incidence of postoperative complications proved 6.53% and 13.04%respectively, which was significantly lower in the plate group than in the tension band group(P<0.05). [Conclusion] Both locking compression hook plate and Kirschner wire tension band achieve satisfactory clinical results for treatment of the simple distal lateral malleolus fractures. By comparison, the plate takes advantages of early weight-bearing ambulation postoperatively and reduced complication over the tension band.
