摘要
目的探讨组织声学结构定量技术(ASQ)不同感兴趣区域(ROI)面积大小对乙肝后肝纤维化程度的研究。方法选取健康志愿者60例及已经肝脏穿刺病理活检证实为肝纤维化的患者60例,每位患者均进行腹部肝脏的二维超声检查及ASQ技术的检查,收集所得的每一位患者的肝脏二维超声图像及ASQ技术图像,将图像导出进行软件分析,所得ASQ技术图像,对其进行不同面积大小的ROI选取分析,得出ASQχ~2直方图。脱机分析软件计算ASQ参数:红色曲线众数(Redmode),将不同ROI下所得参数值分别与肝纤维化(HF)病理分期进行相关性分析。结果健康志愿者中小ROI组与大ROI组红蓝曲线走行均平滑,红线峰值略偏向100cm^2位置右侧,两图未见明显肉眼性差异; HF患者中红蓝曲线走行粗糙不平整,红蓝曲线下面积增大,蓝线峰值增大,偏离100 cm^2,其中小ROI组较大ROI组程度更甚; Redmode值分别在A0、B0、C0各两组间差异无统计学意义(P> 0. 05); Redmode值分别在A0和A1、B0和B1、C0和C1组间差异有统计学意义(P <0. 01); A1、B1、C1组Redmode值均与HF分期呈显著正相关性,相关性系数分别为(r_(s1)=0.678,r_(s2)=0. 824,r_(s3)=0. 635)。结论 ASQ技术可较为准确的评估乙肝后HF,且不同面积大小的ROI均与HF分期正相关,ROI面积为2*2cm^2时,相关性最高,意义最大。
Objective Discuss the research of acoustic structure quantification technology in diagnose liver fibrosis under dif- ferent ROIs.Methods 60 healthy volunteers and 60 patients with histologically proven to be liver fibrosis were examined by B-Mode ultrasound with ASQ software.Images were collected and analyzed by ASQ software offline.Selected ROIs with different areas,and ROIs were analyzed.Then we got their x^2 histograms and values of Redmode.After that,the correlation between the values of Redmode under different ROIs and the pathological results of liver fibrosis was evaluated.Results In the healthy group,Red histogram curves and blue histogram curves were all smooth and the peak values of red curves were all close to the right side of 100cm^2 under both large and small areas of ROIs.It haven't shown a naked-eye difference between groups of large and small ROIs.In HF group ,Red histogram curves and blue histogram curves in the horizontal distribution were more rough and broader,the areas under red and blue curves were much more larger than the red curves,the peak values of blue curves became larger and deviated from 100 cm^2.The situation was more severe in small ROIs groups than in large ROIs groups.The values of Redmode didn't show statistical difference in every two groups of A0 and B0 and CO group (P >0.05);The values of Redmode showed a statistical difference between A0 and A1 group,between B0 and B1 group and between CO and C1 group relatively(P <0.01 );The values of Redmode were all positively correlated to liver fibrosis grade in A1,B1,C1 group,in which,the correlation coefficients were(rs1 =0.678 ,rs2 =0.824,rs3=0.635 ).Conclusion ASQ technology can evaluate the stage of HF after infected with HBV.And the results acquired from different ROIs were all positively correlated to liver fibrosis stage,when the area of ROIs came to 2*2cm^2 ,the correlation and importance were both the highest.
